The patient centered care concept support the active involvement of the patients and their families, in the decision making process that are related upon the treatment services. The IOM (Institute of Medicine) defines patient-centered care as: "Providing care that is respectful of and responsive to individual patient preferences, needs, and values, and ensuring that patient values guide all clinical decisions." The patient centered care is one of the main concepts to achieve an overall goal of health advocacy, in attributing to the safer medical system, whilst involving the patient interaction and involvement in the health care delivery and design. The patient centered care involves the integrating concept to educate the consumers about their health conditions, whilst guiding them to unbiased information about the benefits and risk of their health outcome. The patient centered approach is mainly focused on the patients and their main attributions and interactions with the health care providers, and their main experiences in the main clinical settings such as the health centers and hospitals. This approach addresses the issues of the quality, and the holistic health care aspects to meet the broader health challenges. A people centered approach is distinct since the patients must be informed and empowered, to promote and protect their personal health. The people centered approach involves a balanced consideration of the values, needs, expectations of the health and well-being of all the constituents that are related to the health care system. It aims to promote the health promotion, to reach the general public to take care of their own health, prior they become patients. The Patient- centered care is a level of quality being attai... ... middle of paper ... ...e health system. Both the patient and the people centered care share several core values and guiding principles that are very similar to each other. According to the World Health Organization the basic values and principles are outlined as follows:- • Having and giving respect to both the human rights and dignity to both the patients using the service, and the health care professionals that are giving out and distributing the health care service. The central role in the health process will aid in the development of the economic growth towards the health sector. The patient centered approach will end all the forms of the discrimination, since the patient is seen as overall human being, independently of the race, gender, that might be different from the other patients. All the patients are included, in the inclusion process, by the transparency and accountability.
As our health care system continues to evolve and become more focused on a preventive and coordinated approach to patient care, we too must progress and create programs that follow such principles. The Patient-Centered Medical Home (PCMH) model follows similar ideologies and recently has gained increasing support. The patient’s primary care physician, who will provide preventive and continuing care for the patient, directs this medical model. The PCMH model of care is comprised of a health care team working together to serve their patient and provide quality care.1 The model works to empower the patient by promoting communication with not only the physician but with the nursing staff, specialists, and other health care providers. Every patient

Patient-centered care is an easy nursing competency for me as now. As a student nurse on the floor, I found that I gave my best nursing care delivery by recognizing that each patient is unique and that each has different needs, values, and preferences. After the end of baccalaureate program, I will be at 9/10 rating and a year after pre-licensure program, I will be very competent on the floor delivering respect and compassion to my
Patient Centered care is a concept where the healthcare providers stand in the patient’s position and think about how the patients want to be treated before navigating into how they themselves want to continue with the procedure. It is a strong commitment for the healthcare personnel to be able to manage and regard the patients as thinking and feeling people with the potential to develop and adjust. Thus, the healthcare team needs to be compatible, open-minded and courteous in order to provide the best care possible for the patients.

This study showed that nurses can communicate well when a patient-center approach is used. There is need within health care for nurses to recognize that patients are more than a task that needs to be completed. That the patient themselves are an important element in their own care. By educating and giving nurses the evidence-based research available they can fill this gap. Continued research needs to be conducted on patient’s experiences of how nurses communicate. Showing us the behaviors that patients place high values on. Thus enabling nurses to use a patient-centered
Treating all patients with dignity, respect, and understanding to their cultural values and autonomy. Each patient comes with their own religious belief. With patient-centered care as health care providers, we have to have ways to work around a patient with different beliefs. Catering to their culture differences and needs is a must in order to fulfill their needs.
To be person-centred, you must always be at the centre of the individuals care plan. The Individuals that you work with you have to make sure that you always put their views first. Therefore, you can’t have one care plan for two different individuals because every individual is different. Every individual that you may work with must be involved in every activity and in every stage of their care plan; therefore, whichever activities you may put up you have to make sure that if the individual is capable for taking part. To meet the individual’s needs and support that is required you must take feedback on how the care plan is working for the individual.
Nursing should focus on patient and family centered care, with nurses being the patient advocate for the care the patient receives. Patient and family centered care implies family participation. This type of care involves patients and their families in their health care treatments and decisions. I believe that it is important to incorporate this kind of care at Orange Regional Medical Center (ORMC) because it can ensure that we are meeting the patient’s physical, emotional, and spiritual needs through their hospitalization.
Person-centred processes: It focuses on providing care with holistic approach, respecting patient’s values and beliefs, and working in partners.
Primary health care is the indispensable care based on the real – world, systematically sound, socially adequate technique and technology which made unanimously available to the families and every individuals in the community through their fully involvement where the community is capable to afford at a cost to uphold at every phase of their growth in the essence of self-reliance and self-government. Primary health care in international health is associated with the global conference held at Alma Ata in 1978; the conference that promoted the initiative health for all by the year 2000. “Primary health care defined broadly at Alma Ata emphasized universal health care across to all individuals and families , encouraged participation by community members in all aspects of health care planning and implementation and promoted the delivery of care that would be scientifically sound , technically effective , socially relevant and acceptable” (Janice E.Hitchcock,2003). Primary health care is commonly viewed as a level of care or as the entry point to the health care system for its client. It can also taken to mean a particular approach to care which is concerned with containing care, accessibility, community involvement and collaboration between other sectors. The primary health care policy has some principals that have been designed to work together and be implemented simultaneously to bring about a better health outcome for the entire society.

patient centered care necessitates that healthcare professionals “acknowledge their patients as an equal partner in the development and assessment of their care.” It was interesting to see how a provider’s attitude can contribute to medication non-adherence in a patient. Nonetheless, I believe providers at their core want their patients to get better because it primarily is an indicator of their competence and success in their chosen profession.
Person-centred care is the treatment and care provided by any health service. This particular concept places the person at the centre of their own care and considers the needs of the individual and the surrounding family members. At the core of this concept is getting to know the patient as a person look beyond the illness and build a therapeutic relationship with both the patient and the surrounding family members and carers. Talking to the patient finding out what their preferences are to the care you’re providing. Working within the person centred approach you should be flexible Meeting patients' individual needs while respecting values, preferences and needs. ,making sure that you keep your patient informed about the treatment that they are receiving.
