A community is a group of people who share some of the same characteristics or common areas in their daily lives. Geopolitical community is a natural community. An example of this would be the city we live in the neighborhood we reside in. Phenomenological community is a community we have selected to be a part of. An example of this would be the church we attend (Nies & McEwen, 2015). Public Health- Assessing our community and monitoring people at risk. Measures taking to prevent disease, promote health, and prolong life among the people in our community (World Health Organization, 2016). Many communities have limited resources to take care of chronic conditions and help with preventive medicine. With the baby boomers aging and retiring there are more elderly who require care for chronic conditions and some of these same people are the providers that used to work with in the communities. Many healthcare administrative employees are working with communities to help improve this access for patients (American College of Healthcare Executives, 2012). Once the elderly have coverage I believe it is imperative to help the elderly be aware of what care is provided as well as what preventive care is needed. As community resources nurses need to ensure their patients truly understand the plan of care and need to constantly reassess the patients for changes in living situations, health, and communications. Saginaw, Texas is located in the Northwest area of Tarrant County. Saginaw is near several major thoroughfares which include DFW airport, Fort Worth, Dallas, and Alliance. Saginaw was founded in 1833 by a man named Jarvis Green (CivicPlus Content Management System, 2016). Saginaw prospered with the transportation dependent industry which included grain storage and milling facilities.
