A case study of Teratogens and Prevention during pregnancy
Conception is an exhilarating moment which marks the commencing first term of pregnancy. To many, knowledge of conception is vital to ensure the safety of the newborn. At this stage, women begin to take prenatal vitamins, they change poor eating habits and some use exercise to make certain their body is ready to deliver a healthy baby. However, not all women take the precautions necessary. Ashley is our motivation for this case study. Ashley, pregnant for the very first time, believes that her alcohol consumption and cigarette addiction will not have an effect on her child.
Sadly, Ashley’s ignorance and disregard for her child’s health can lead to a host of irreversible side effects. Mothers who consume alcohol during pregnancy often give birth to children with Fetal Alcohol Spectrum Disorder (FASD). FASD has a plethora of cognitive and central nervous system defects but may go unnoticed until critical periods for the child are not met. Initially children born with FASD may have abnormalities linked to vision, hearing, renal failure and cardiac deficiencies (Carter 7). In addition to the preceding, some children born with a form of Fetal Alcohol Syndrome show dysmorphic facial features such as a smooth philtrum, thin upper lip and small eyes (Kyigne 296).
In addition to the physical shortcomings that alcohol during pregnancy can cause, cognitive defects are countless. Children who are born to mothers who drink while pregnant display inadequate intellectual abilities in areas such as attention, memory, learning, linguistics and number and visual processing (Greener 118). Childbirth is nothing short of a miracle. The placenta—the organ connecting a developing fetus to the uterine wall and allowing for waste elimination, nutrient uptake and gas exchange via the mother's blood supply—filters most harmful substances that threaten an embryo, though some may still pass on to the fetus. These harmful substances, called “teratogens,” range from environmental chemicals to the passing of maternal diseases, and can negatively impact the normal developmental cycle of a fetus. The title “teratogen,” however, effectually refers to any substance or chemical exposure with the potential to cause birth defects in prenatal development. Exposure to teratogens can result in a broad spectrum of physiological and psychological issues in later life, including malformations of the body. (Malformations resulting from exposure to one of the most common teratogens—alcohol—can be observed notoriously in Fetal Alcohol Syndrome, or FAS, where patterns of mental and physical defects develop in association with high levels of alcohol consumption during pregnancy.) Though the toxicity of these teratogens is particularly damaging during the fourth through tenth weeks of gestation, teratogens can harm throughout the span of development in the womb.
There is no known safe amount of alcohol a woman can drink while pregnant. Alcohol can cause life-long physical and behavioral problems in children, including fetal alcohol syndrome. FAS is a serious condition where babies can be born with mentally ill condition and may have deformation mainly in there face. When you consume alcohol during pregnancy, so does your baby, because alcohol passes freely through the placenta to your baby. If you choose to drink alcohol while you are pregnant, you will increase the risk that your baby will be born with a Fetal Alcohol Spectrum Disorder (FASD). Fetal alcohol syndrome is a condition in a child that results from alcohol exposure during the mother 's pregnancy. Fetal alcohol syndrome causes brain damage and growth problems. The consumption of alcohol during any gestation of pregnancy equates into alcohol fetal consumption, which can cause detrimental physical and neurological defects. Infants born with the varying degrees of detrimental effects of prenatal alcohol ingestion can have an array of disorders which are described as Fetal Alcohol Spectrum Disorders [FASD] (May & Gossage, 2011a). The most profound effects of prenatal alcohol exposure are on the fetus’s brain development, which includes cognitive and behavioral effects that follow (Riley, Infante, & Warren, 2011). The incidence of FASD is believed to range from 0.2 to 2 per 1000 live births (Douzgou et al., 2012). Alcohol yields teratogentic effects in all the gestations, with peculiar features in relationship to the trimester of pregnancy in which the alcohol is consumed (Paoletti et al., 2013). Due to the fact that there is no exact dose-response relationship between the amount of alcohol ingested during the prenatal period and the extent of damage caused by alcohol in the fetus, abstinence from alcohol at conception and during pregnancy is strongly recommended (Paoletti et al., 2013).

When it comes to pregnancy, expectant mothers usually have a lot of questions and concerns. One such concern is alcohol consumption. Some people feel that it is okay while others are against the consumption. However those who drink take a huge risk that can result in what is referred to as Fetal Alcohol Spectrum Disorders (FASD). This umbrella term is used to describe the range of damage from alcohol exposure to a fetus. The characteristics, diagnosis, and the mother are all important factors to look at when trying to understand these birth defects.
A mother who drinks while she is pregnant stands a high risk of harming their unborn child because the alcohol passes through her blood to her baby, and that can harm the development of the baby’s cells. This is most likely to harm the baby’s brain and spinal cord. Many of the common effects of a child suffering from fetal alcohol spectrum disorder (FASD) is: distinctive facial features, growth problems making them smaller than the average child, and learning and behavior problems.
Alcohol (wine, beer, or liquor) is the leading known preventable cause of developmental and physical birth defects in the United States. When a woman drinks alcohol during pregnancy, she risks giving birth to a child who will pay the price, in mental and physical deficiencies, for his or her entire life. One study (Phyllis Trujillo Lewis, MA, Philip A. May, PhD, and Virginia C. Shipman, PhD, 2007) asserted that “Numerous studies on alcohol-related birth defects have concluded that maternal drinking, compounded by other risk factors, leads to fetal alcohol syndrome (FAS). FAS is a serious birth defect and the most common non-genetic cause of mental retardation” as said by (Hankin, 2002; Abel & Sokol,1986; O’Connor, Kogan, & Findlay, 2002; May & Gossage, in press). It is unknown how much maternal alcohol consumption results in FAS or other related disorders, or why some women who drink are at substantially higher risk of giving birth to a child with alcohol-related disabilities than others (Stratton, Howe, & Battaglia, 1996). However, researchers have identified several maternal risk factors differentially associated with FAS. These include advanced maternal age, number of pregnancies, previous births of a child with FAS, cohabitation with a male partner who drinks heavily, and low socioeconomic status (SES; May et al. 2004; 2008a; Viljoen et al., 2002). FAS is 100% preventable, which makes awareness and education the core preventative method for FAS. It is seen through Lewis, May & Shipman’s research that women who are less educated are less aware of the risks involved with drinking while pregnant.
Children can be effected by their caregiver’s alcohol and substance abuse in numerous ways. Fetal Alcohol Syndrome is a pattern of birth defects caused by maternal consumption of alcohol during pregnancy. “I brought you into this world, and I can take you out!” A child has most likely heard that phrase at some point in their life. Although, it is not ethical or legal for a mother to “take her kid out of this world”, it does bring up a good point that it was through her body, that the child was born. One of the most important responsibilities in this world is a mother carrying a child in the womb. There are many divine processes that take place during gestation, but there are also many contributing factors from the mother that can affect the developing human. These factors may include what a woman ingests and exposes her embryo or fetus to. Sadly, alcohol use during pregnancy is an ongoing problem that can have detrimental affects on the fetus, including Fetal Alcohol Syndrome (FAS). Choosing to drink alcoholic beverages during pregnancy is a choice, a risky choice. Unfortunately some women don’t even know they are making a risky choice by consuming alcohol because it is in the early stages of pregnancy. It is common for a female to not find out they are pregnant until at least the fifth or sixth week after fertilization. In 2006, 49% of all pregnancies in the United States were reported unintended on a national survey.1 The highest rate of preventable birth defects and mental retardation is due to alcohol use.2 In this paper, I will further discuss FAS, the potential effects of binge drinking during the embryonic stage of gestation, and what actions need to be taken in order to reduce the incidences of alcohol related birth defects.

Most women have heard that smoking is very deadly to the human body and can cause cancer, heart disease, and other major health problems, yet it is still an ongoing problem. Around the world, about 250 million women use tobacco every day and this number is increasing rapidly, according to data presented at the 2009 14th World Conference on Tobacco or Health in Mumbai (March of Dimes , 2011). Not only do women smoke, some choose to continue to smoke while pregnant. Smoking during pregnancy is a worldwide problem, although it is more common in developed countries such as the US, where an estimated 18 percent of pregnant women smoke. In developing countries, it 's estimated that only 8 percent of expectant mothers smoke. These percentages may sound low, but together they equal up to one million babies born worldwide each year to mothers who smoked while pregnant (Smoking during Pregnancy , 2009) . One study found that about one in four women who smoked while pregnant deny it. Maternal smoking during pregnancy increases the risk of birth complications and has long-term developmental consequences for child development, including deficits in general intelligence, academic skills, and cognitive functioning. As social inequalities in smoking have increased over time, maternal smoking during pregnancy has become concentrated among women with lower levels of education (e.g., more than 20% among women without a high school degree) (Gilman, Breslau, Subramanian, Hitsman, & Koenen, 2008). Despite the warnings about the dangers of smoking while pregnant, some women still choose to smoke which places themselves and their baby at risk for many health issues.
Every woman when pregnant has a 3-5% chance of having a baby born with a birth defect, and these chances increase when the developing fetus/ embryos are exposed to teratogens, whether it’s intentional or unintentional (Bethesda (MD), 2006). Teratogens can cause severe birth defects, malformations, or terminate the pregnancy altogether (Jancárková, & Gregor, 2000). The placenta is known as an effective barrier from any detrimental pathogen that can potentially hurt the fetus. The timing of exposure of any teratogen is critical to the impact of prenatal development (Bethesda (MD), 2006). The most vulnerable time of the fetus for severe damage is during early pregnancy when all the major organ and central nervous system (CNS) are developing. Miscarriages have an important role in keeping a pregnancy from evolving when there is something serious going on with the developing fetus/embryo. Miscarriages are more common than we think and are the most familiar type of pregnancy loss (Bethesda (MD), 2006).
