Recommended: Detailed intervention plan social work

I am currently finishing my practicum at Mason and Partners Free Clinic. The clinic uses a bridge-care model that provides health care, school physicals, screenings, and mental health services for vulnerable populations located in low income and medically underserved areas. Social work interventions are brief as patients get referred to proper treatment facilities such as Community services Board and Evergreen. Intervention: Mother of 12 year old reported that her daughter was having trouble in school as she could not concentrate and was acting out as she did not want to speak to her mother. Child reported feeling sadness, low mood and instances of crying for no reason. Child reported that her mother was not supportive or understanding of her
