Problem list:
• Grade 2 strains of distal right biceps femoris.
• This is the second time he has injured his right biceps femoris.
• He has a history of left knee MCL sprain.
• Employed as a full time soccer player and therefore his family will depend on him for money
• Two young children that depend on him financially.
• Will his club continue to pay him even though he is injured?
• He will find it hard to carry out every day activities that are expected of him being a father of two young children. This will put more pressure on his wife.
• For the majority of the season he has only been coming on as a substitute even though he is team captain. This will have a negative psychological effect on John.
• It will be a challenge to maintain his fitness during his rehabilitation because John will be likely to adopt a negative psychological mindset due to his current circumstances.
• It is the second time he has been unable to play for his team due to a biceps femoris strain.
• He is at risk of losing his place as team captain if he doses not return to form soon.
• He is going to be under pressure to return to his team as soon as possible and therefore is at risk of going back to play without having fully finished the muscle rehabilitation program set out to him.
• Ultimately his career, family and employment depend on his recovery from this injury.
Short-term goals
The short-term goals for John are to resume playing football as soon as is safely possible to do so. To achieve this he will have to be given an accurate rehabilitation program and follow it strictly, he will have to attend physiotherapy on a daily bases. The patient is on antibiotics to prevent infection. For a full result and return, the patient will have to take several months to a year for strength and range of motion to return. As some pitchers and other main players might have to take up to two years off to make sure they are the way they were before they had Tommy John surgery. Physical Therapy is a must after surgery. After returning home, most of the patients will have an exercise program for gentle range-of-motion. There are three different stages of rehabilitation. The first stage for a pitcher is gentle motion exercises while wearing an elbow brace; after 6-8 weeks, the second stage is elbow strengthening exercises; and the third stage is to start tossing a ball and gradually working from the mound. Controlled increases in physical stress through progressive resistive exercise cause muscle fibers to hypertrophy and become capable of generating greater force.3 Early emphasis is on restoring joint range of motion and muscle flexibility, however, resistive exercises are not delayed. The initial emphasis of muscle loading should be on endurance, accomplished with lower loads and higher repetitions. Progressive resistive exercises are initiated at the available range and progressed to new positions as wrist range of motion returns in all planes. Both the overload principle and the SAID Principle (Specific Adaptation to Imposed Demands) are important considerations in therapeutic exercise dosing.1,3,11 Within pain tolerance, dosing progressive resistive exercises that maintain a therapeutic stress level will encourage muscle tissue hypertrophy. Finding activities that produce the correct force and repetition, without injury, is the goal of the remobilization period. Starting with low force, moderate to high repetitions, and encouraging therapeutic rest following induced stress is important to both the overload principle and the SAID Principle. Athletic trainers are mainly responsible for injury recovery and injury prevention. However, these responsibilities are physical concerns, but what about the mental concerns? There is a movement within the athletic training field to approach sport injuries with a more holistic perspective. Many athletic trainers have had inadequate education in sport psychology to institute any sort of psychological initiative in their rehabilitation programs. These types of injuries could take upwards to eighteen months to return to playing condition, and in this time an athlete will often fall behind in the sport that they love. This often causes the athlete to quit the sport they once loved because they can no longer truly compete amongst their peers. I know that there has to be a way to accelerate this process of recovery and get those athletes back on the field. This is where my passion for orthopaedics
Alison will need to take the coaching approach which is a high directive and high supportive behavior. Involving John in the decision making process and seeking out his input will encourage and motivate him to commit more to the project. Alison will also need to provide the support he needs, such as resources to help John succeed in his new role with the team. Motivation and praise will also help John build his confidence.
What researchers and experts must do is not only discover the cause, but also design a special preventive program for female ACL injuries. Many researchers have found that different anatomical structures and neuromuscular imbalances make females more susceptible to ACL injuries. A special training program emphasizing hamstring strengthening could be effective in reducing the risks of acquiring ACL injuries for female athletes.

Occupational therapy was a career choice I fell into. When I first arrived to Keiser University back in 2009 I was going to apply for the physical therapy assistant program. Physical and speech therapy where the only therapeutic disciplines I had any knowledge of at that time. When I met with the admissions counselor she informed me the waiting list for the PTA program was about two years, I was floored I wasn’t going to wait that long I needed to start school ASAP. The counselor then asked me “ what is your goal?” and my response was “to work with children in the medical field without being too medically involved”. High school football players sustain a major proportion of season injuries. A major part of these regions are due to ligament sprains, targeted stretching exercises may be beneficial. The most injured players were those with the position of running back and linebackers. In the 2005-2006 season there were more than half a million injuries nationally of high school football players.
