Patient-centered primary care is beginning to form the cornerstone of quality health care, although there remains to be a separation from ideal patient care and the reality of the situation. Health care services that are focused on the specific needs of each individual are an important step forward to making an improvement in overall health outcomes and patient satisfaction with health services.
Dimensions of Patient-Centered Care
According to the Picker Institute, there are eight8 primary dimensions of patient-centered care. There These were originally created with hospital care in mind, but are equally relevant to primary care services. They are:

Adequate communication among the primary care team members is also important, including sharing of patient information, so all health professionals have access to the most up-to-date records. Vision For Patient-Centered Primary Care Practice
A “2020 vision” for health care in America has been expressedconveyed to include access to services, patient-centered care, decisions based on scientific evidence and affordable health insurance for all individuals. Additionally, this vision emphasizes a commitment to the continual improvement of the health sector to lead to better public health outcomes.
Patient-centered care lies at the core of this vision. The benefits of tailoring health care decisions to the individual patient are well recognized, and the medical sector is looking to take the future of the profession in this direction.

The Patient-Centered Medical Home (PCMH) model follows similar ideologies and recently has gained increasing support. PCC allows the patient to have autonomy and a more collaborative role in making decisions regarding their treatment.
In nursing, it is important to understand the difference between the different developmental groups for pediatric patient’s and how these differences affect the care and guidance that patient receives.

To be able to have health care that is functional and effective it starts with primary care. “A robust primary care system is the cornerstone for a more equitable health care system” (Fiscella, 2011). Restructuring of this program in certain areas is important “payment reform, enhancing the training pipeline, transforming practice, and buttressing the primary care safety net” (Fiscella, 2011).
Nursing should focus on patient and family centered care, with nurses being the patient advocate for the care the patient receives. Patient and family centered care implies family participation. This type of care involves patients and their families in their health care treatments and decisions. I believe that it is important to incorporate this kind of care at Orange Regional Medical Center (ORMC) because it can ensure that we are meeting the patient’s physical, emotional, and spiritual needs through their hospitalization.
Person centred care is defined as health care professionals work together for people who use the health care services. Person centred care also helps to support the patient’s knowledge and also helps the patient to develop an understanding of their health condition and also gives them the confidence to effectively manage and make educated decisions about their own health and also the health care in which they receive. (Health Foundation 2014). This suggests that each individual needs to be treated with the same amount of respect and they also need to be treat equally. Conversely, the introduction of “patient-centered care” has been pushed immensely, to ensure that patients and families feel they get the medical attention they are seeking and paying for. Reforming the health care delivery system to progress the quality and value of care is indispensable to addressing the ever-increasing costs, poor quality, and increasing numbers of Americans without health insurance coverage. My most valuable work with patients is to listen and learn from each one, and let them determine their goals.
