I have been a nurse for over twenty years and have worked in different areas of nursing. I currently work as a Lead Complex Care Manager for the Population Health/Patient Care Medical Home Model Program (PCMH). I provide disease management education and coordinate community services for patients with chronic complex conditions. In my role as a Complex Care Manager, I have participated in project initiatives such as identification of patient gaps in care.

In 2010, I was able to obtain my Masters in Adult Health Nursing at Lehman College, while working full-time and raising pre-adolescent teenagers. One of the most challenging experiences I found while working as a medical-surgical nurse on the floor was the lack of time spent interacting with patients through teaching and treating them holistically.

In psychiatry, I gained insight into how mental illness can impact patients not only mentally, but physically as well. In case management, I am constantly engaging patients regarding management of self-care and witnessing the rewards of such collaboration between patient and “nurse”.

My determination to treat patients holistically, not only on the clinical aspects but on the social determinants impacting their health outcomes led me to obtain my Case Management Certification in 2016. The preparation for the CCM Exam was grueling, yet made me more determined to pursue my objective of becoming a Family Nurse Practitioner.

The FNP Program has been a long-life dream of mine. The Family Nurse Practitioner collaborating with the Primary Provider is at the heart of Family Practice. They are the gatekeepers that drive the healthcare experience of the patient. I believe that I would serve a great value to this new
