In the case study, it starts out talking about a couple Juan and Maria who were immigrants from another country that came to the United States to move with their daughter (Carla), her husband (Soloman), and two grandchildren. Maria has diabetes and she pays for her insulin out-of-pocket, she’s worked as a housekeeper where they were paid cash. Juan has high blood pressure and high cholesterol, he receives help from the federal health center for his prescription. He worked as a janitor, at his job they were paid by salary, received Social Security and had a small amount of money put into his 401(k). On the other hand, neither of their jobs provided them with health insurance.
Later in the case, Juan had a stroke and had to retire from his janitor
When Soloman and Carla are at work and the grandchildren are at school Juan and Maria can go to this program and have a productive activity filled day. A home health aide program allows a trained aid to come in and mentor a client while family members are at work or school. Since Solomon feel that taking care of Juan and Maria are a distraction for his children this would be a great way for them to continue to live at home without putting stress on the children to look after them. PACE (Program of All-inclusive Care for the Elderly) is program sponsored by a Medicare and Medicaid program. It 's helps people meet their health care needs in the community (residence) instead of going to a nursing home or other health care facility. Since Maria is against her and her husband going into a nursing home this is a great program the will teach them how to live independently. All of the listed problems could be resolved by placing Juan in a nursing home. Since Maria is in fairly good health besides her diabetes I do not see the need of her to be in a nursing home beside to accompany her husband. At a nursing home the can help Juan regain his independence by professional teaching and strengthening him on how to improve and live with his disabilities. The only other alternative is to seek help through the PACE program, adult day, and by hiring a home health
The cultural identity that contributes to the poor health outcome of Senora Vasquez is that initially she doesn’t want to receive treatment from the hospital upon having a burned leg. Since they don’t have enough money to support the treatment Senora Benita has stop drinking the antibiotics and changed the dressing for only few times which makes the site become more infected. Prior to that, upon having the burns she put some lard or butter to the site and some herbs, which is not proven to be helpful in treating burn areas this lead to infection of the patients burn site. Also, self-care beliefs, the tendency of using home remedies, which was perceived as minor problem, was not given attention

Many times nurses struggle to maintain a patient deteriorating in front of them all the while make a multitude of calls to the physician for orders or concerns. Having a set of “expert” eyes assisting you in these times helps alleviate stress and encourages collaboration amongst staff. (Parker, 2014)
“We can’t turn away from a patient’s pain just because it’s difficult” (chapman, 2015, p. 88). I know the path of least resistance is taking a path of ignorance. Easy, is to ignore or neglect the true pain patients experience in times of crisis. As caregivers I believe we all want to heal others or we wouldn’t be in the field of nursing, but there are only handfuls willing to be present during the healing process because sharing one’s pain hurts. As a surgical nurse, I find being genuinely present takes hard work on my behalf, not only mentally but emotionally. On a unit where patient’s needs and conditions are changing at astonishing rates, being present requires mental strength in order slow down enough to recognize the value presence
There are many who believe that the next shortage will be worse and the demand for nurses will increase. There will be more jobs available especially with the baby boomer nurses retiring. Wood believes that when nurses retire, the next shortage could be even worse than the previous shortage. According to Wood this would lead to an “intellectual drain of institutional and professional nursing knowledge” (Wood, 2011, para 15). I have been a registered nurse for the past six years. I started my nursing career in a long-term care facility where I worked for a year and half . I always wanted to challenge myself so l left long-term care and went to work in the intensive care unit for four years where I saw how people with diabetes are suffering when the disease in not managed well. They have worked all of their lives to secure retirement, but their retirement has been threatened because of the rising cost of long-term medical care. Insurance companies have failed to provide affordable long-term care, protection that most senior citizens need. This lack of long term care and affordability has been a serious problem for the health care system. In some cities, the shortage of hospital beds is so serious that it is common for patients to stay in emergency rooms before they can be admitted to an inpatient room (Drake 109). More than one thousand hospital beds are occupied by people who could be better care for in nursing homes or through home health care (Drake 110).
