Some instructions to follow:
1. Housing Search Tracker: Date when you mail out each application, list the landlord/managing agent, their email/phone number. Send a copy of their results back to Hildebrand. We have to keep track of all your housing efforts. Pay very close attention to all lottery application deadlines.
- Add any new applications to this list and follow the formation outlined
2. Monthly Budget Worksheet: (Due 1/9/18) List any income you receive, SSI Wages etc. send us a recent copy of all your family’s income and again when it changes.
- Include any rent you may owe to previous landlords, this way we can assist you eliminating that debt prior to exiting shelter
- Include any utility debt (gas/electric) and the addresses associated with these debts if any

4. Authorization to Obtain/Release: I included additional blanks in case there are other people/organizations you feel we should be in contact with to assist you while you are here.
5. CORI: (Due 1/9/18) Please thoroughly fill out the CORI that is required of all participants and return that in the self-addressed stamped envelope. The sealed record will be waiting for you to share with us. (Do not sign where it says employee, fill out the section above)
6. Five-Year Housing History: Please fill this out starting with Itasca followed with where you stayed previous to that, maybe it was a hotel provided by DHCD, then previous to the hotel, where were you, etc.
7. Self-Assessment & Assessment Form: (Due 1/9/18) Thoroughly fill out these two forms based on knowledge of yourself and return in the envelope
