What is the 8th leading cause of death in USA? (Please do not guess, you will find in the end of this paper!) First , few horrific dots: 44,000 americans die each year as a result of medical errors, more people die from this reason than motor vehicle accident , breast cancer or AIDS. Total national cost calculating as: lost income , lost production , health care cost are estimated as 17-29 billion $$$ and this is modest estimate that is not including the preventable adverse drug event and repeated diagnostic test . But not all the cost can be directly measure: loss of trust in medical system , diminished satisfaction for both patient and health care providers, shortage in high quality care provider due to reduce enrolment to universities and lower level of population with good health status. Yet there is silence surround this issue, nothing is exposes to the pubic eyes, anecdotal cases are not delivering the pain of a family or a child. In this report:”To Err is Human”, safety define as freedom from accidental injury and first time define the safety goal from patient perspective. Error is defined as failure to planned action to be completed as intended or the use of wrong plan to achieve an aim, errors can happen in all stages of care from diagnosis to treatment and to preventative care. Since it is a planned intervention in medical system the focus must shift from blaming individuals in past errors and focus on preventing future errors by implementing safety into the system. Use information technologies ready and friendly to patient and care team. Manage growing knowledge. Coordination of care across patient condition , services. Improve team work. The final challenge is everyday life: Incorporating care process and outcome measures into their daily work. System in transition to the 21st century need to be ready and prepare the workforce, at least three approaches can be taken to support the transition: 1. Redesign the way health care trained, including evidence base learning/practice, using multidisciplinary approach. 2, Modify the ways in which health profession are regulate 3. Examine how liability system can constrictively support changes in care delivery.
