PROPOSED INTERVENTION Intervention Proposal for Wheatbelt, Western Australia (WA) case study: Cardiovascular Health and Rehabilitation Program. It is a program that includes an exercise and education program for people suffering from cardiovascular disease (Australian Cardiovascular Health and Rehabilitation Association (ACRA), 2011). It provides patient education about how to manage cardiovascular disease, secondary prevention, behaviour changes, and psychological support, medications, exercises, and diet, social and emotional issues. It consists of three phases (ACRA, 2011). • Phase 1: Hospitalisation. The benefits of this program would be measured by cardiac education booklet which includes exercise diary, diet plans and goal setting (CRNB, 2008). This objective would be achieved by checking the booklet every 2 weeks, so that the patients can be guided if needed any support. For the patient in phase III, records will be measured via telephone conversation (ACRA, 2011). According to Mampuya (2012), the studies showed that the exercise-based cardiac rehabilitation reduced the mortality rate by half as well as reduced the major risk factors, particularly smoking. Other factors improved by this program are reduction in inflammation, ischemic preconditioning, a more favourable fibrinolytic balance and endothelial function improved (Mampuya, 2012). A meta-analysis showed that patients attending cardiac rehabilitation program showed 47% reduction of mortality in 2 years after the program and 17% reduction of myocardial infraction reoccurrence at 12 months (Mampuya,
