Effective health care collaboration has been valued as a key strategy for the health care reform. The health care collaboration not only improves the outcomes of medical care provided to the patient, but also provides benefits to health care providers. In addition, the team members often see the same medical situation from different angles. For instance, it may happen to a team with attending physicians from different departments. It is understandable that team members with different expertise and standpoints have divided opinions on the same medical situation. A medical team often deals with emergencies. Sometimes, the team members respond to the emergency medical situation with a reflexive reaction, which results in insufficient team communication in decision-making. It is obvious that a great deal of interprofessional research has been aimed to educate practitioners and nurses over the past decade for interprofessional practice (Orchard, King, Khalil & Beezina, 2012). The Institute of Medicine (IOM) “The Future of Nursing Leading Change, Advancing Health” (2010) recommend that private and public organizations, nursing programs and associations increase opportunities for nurses to lead and manage collaborative teams. Health care reform has created a shift in the healthcare delivery to place more emphasis on interprofessional health care teams (Sinfield, Donoghue, Horobi & Anderson, 2012). New implications are directed towards continuing education for health care workers to understand the meaning of interprofessional collaboration to support the changes in collaborative practice to improve patient outcomes (Orchard et.al, 2012). Encouraging health care professional to collaborate as a team more effectively may seem as the answer to improve the quality of care, but ineffective communication from team members to collaborate on the care needs often attributed to patient safety issues. Consequently, even when professional collaborative teams work together, there is no means to validate and measure the impact on continuing education for nurses about interprofessional collaborative practice (Sinfield, et al., 2012).
It is true that as medical students we may never have to make decisions regarding the treatment plan of patients and that we will always be supervised when carrying out procedures. The chances of inflicting harm on patients is quite slim as a result of this but in the rare cases that it happens we should abide the duty of candour and inform our supervisors immediately. Since the role of a medical student is quite trivial in a multi disciplinary it is often the responsibility of the team leader to inform the patient of the error (4). Effectively coordinated and collaborative inter-professional teams are essential to the care and treatment of patients (Rowlands & Callen, 2013; Doyle, 2008; Ruhstaller, Roe, Thürlimann & Nicoll, 2006; Simpson & Patton, 2012, p. 300). Communication is a process of conferring information between individuals through use of speech, writing or various other means, and is critical to the success of a multidisciplinary team (MDT) (Higgs, McAllister & Sefton, 2012, p. 5; Rowlands & Callen, 2013; Sargeant, Loney & Murphy, 2008). An MDT must use multiple strategies to enhance communication and ensure their success (Doyle, 2008). An effective MDT generates opportunities that benefit healthcare, which is the reason for the recent dominance of inter-professional care in health practice (Simpson & Patton, 2012, p. 300; Rowlands & Callen, 2013). Many barriers prevent effective communication within inter-professional teams. Lack of communication within MDTs presents challenges to their success, leading to numerous consequences, including the failure of the MDT (London Deanery, 2012; Sargeant et al, 2008). Communication between professionals is the key factor underpinning the potential success or failure of inter-professional teams, the outcome of the functioning of MDTs will either benefit or impair care of patients.
“Physicians and other health care professionals all agree on the importance of effective communication among the members of a health care team. However, there are many challenges associated with effective interprofessional (between physicians and other health care providers) communication, and these difficulties sometimes lead to unfavourable patient outcomes” (Canadian Medical Protection Association, 2011 p. 11).
This article points out many of the techniques that were talked about in the IPE Tegrity lectures. Some interventions that are used to increase communication skills are provide effective interprofessional communication are the SBAR, team huddles, and multidisciplinary rounds using daily goal sheets. To briefly describe each, the SBAR is a standardized means for communication in patient situations. It asks these four questions, “What is going on with the patient,” “What is the clinical background or context”, “What do I think the problem is”, and “What do I think needs to be done for the patient”(Dingley, 2008). This is used by all health care disciplines, hereby ideally eliminating hierarchy between the physician and the other disciplines. Team huddles are quick pow wows that set the flow of the rest of the day. In the article, it states that these team huddles result inlet interruptions during the rest of the day and immediate clarification of issues (Dingley, 2008). They have specific guidelines to make sure that they run as effectively as possible as well. A minor with asthma experiences difficulty adhering to controlling medication. Noncompliance is common, as Zullig et al. (2013) state that on average 50% of medications for chronic conditions are not taken as prescribed (p. 2611). Patient was recently hospitalized for an asthma attack that could not be controlled with rescue medications; while in the hospital, he indicated that he was not taking his prescription. He does not refill prescriptions and does not attend follow-up appointments. Patient portals provide the technology to improve both of these aspects to create the most effective and personalized care for the patient. Successful communications strategies are imperative to health care organizations because the patient’s health depends on it. Patient portals create an efficient environment in which the patient feels connected to their health care provider and communication is effectively driven through interaction both in and out of the office.
In this event, the matter that is unusual can be the fact that I have experienced and witnessed the process for interprofessional collaboration between the community nurse and other professionals that I have never knew about before. This event made me realize that there are many aspects of community nursing that I have knew about before where in this situation it is the importance and accountability of interprofessional collaboration. I have explained three of the seven principles that I use while interacting with my patients daily. The three methods of communication were defined and explained how each is used in the healthcare setting. The communication method that works best on my unit was explained per my unit’s preference.
