Population health, what is it? The term is exactly what it sounds like, the health (more so health status at outcomes) of the population, whether it be by city, race, or age. Population health works to identify and remedy health determinants that affect different populations. These determinants can be social, economic, physical and individual/biological. The study of population health will then research the different types of health determinants and identify past, current and possible future health outcomes to improve the overall quality of health and healthcare. The VHA has seen a rise in veterans who aren’t receiving the appropriate care. This may be due to a number of reasons, some of which being long wait times, distance from care centers (access), or the affordability of care. Working to reform healthcare, the VA needs to implement both short and long-term solutions, thus the implementation of population health. Population health will allow the VHA to identify what health disparities are occurring in different populations by gathering patient information at their visits. By gathering data, both patient information such as BMI and location information, the VHA will be able to start identifying trends and reoccurring issues by analyzing the information. As a field, healthcare is currently working to build larger datasets, interoperability and user interfaces. These technologies alone can improve the information and process of gathering datasets. Just thinking back to the case studies completed this semester, even if full interoperability isn’t reached, data mining, federated databases, and data harmonizing allow for researchers to pull information from different sources without compromising the security and privacy of patients. This would require standards, foundational framework, and user participating resulting in uploaded datasets that can then be used for research in different ways. Lastly, the data that has been used for existing studies has been conducted in highly populated areas, not providing much information on rural areas, or areas in which access to healthcare is not as easily available. Public Health plays a big role in our society and has different duties that need to be carried out. The purpose of Pubic health is to help prevent epidemics and spread of disease. Public Health is supposed to help protect against environmental hazards and prevent injuries. They also promote and encourage healthy behaviors. The 10 Essential Public Health Services helps provide a foundation for any public health activity. These services provide a structure for Public Health and maintain a healthy environment for citizens. Each service helps fulfill different duties to ensure quality improvements in the public health systems. The public health services help as a foundation for the National Public Health Performance Standards. The three essential services are to inform, educate, and empower people about health issues, develop policies and plans that support individual and community health efforts, and link people to needed personal health services and assure the provision of health care when otherwise available.
It is the interrelationships among these factors that determine individual and population health. Because of these, interventions that target multiple determinants of health are most likely to be effective. The accuracy of matching patient records becomes troublesome when looking at their demographics. “Different systems use different demographic information to match individuals to their health records,” thus it can cause a staff member to identify the wrong patient [4]. Furthermore, since state privacy rules vary, complications can arise when trying to share patient data from one state to another. Unreasonable high “system costs and legal fees can deter providers” from fully achieving interoperability also [4]. Healthy People 2020 (HP 2020) is an extensive combined effort by the U.S. Department of Health and Human Services, Federal Agencies, Public Stake Holders, and an Advisory Committee that utilizes the Social Determinants for Health as its framework, to aid the transformation of the United States to a society in which people live long and healthy lives.
