History of Tuberculosis
Abstract
This paper discusses the historical aspect of the fatal and contagious disease now known as tuberculosis. It discusses the history of Tuberculosis beginning with Ancient Egyptian mummies to the most current discovery of TB. Greek philosopher such as Hippocrates and Plato observed the first cases of tuberculosis. They are two out of the many people that gradually discovered the TB out of the many. It talks about the different remedies that different philosophers and physicians came about that have been revised.
Tuberculosis, also known as “the white plague,” (The history of tuberculosis 2) is a silent and unknown prehistoric air borne disease that quickly spreads and kills nearly two million people worldwide (Tuberculosis-Revolution). Today, tuberculosis is the most deadly disease known to man, more deadly than HIV/AIDS. Despite advances in treatment, TB is a global pandemic, fueled by the spread of HIV/AIDS, poverty, a lack of health services and the emergence of drug-resistant strains of the bacterium (Tuberulosis-revolution Health). However, TB has an unknown past, and in order to understand tuberculosis, one must know its history.
Evidence of Mycobacterial infection has been found in the mummified remains of Ancient Egyptians 1000 B.C. The “Father of Medicine,” Hippocrates (460-377 B.C), made the first breakthrough in TB research. He named the disease “phthisis,” which means the “shrinking of the body,” because the majority of the cases he found occurred in the spinal area. One of the largest epidemic events in history, the Bubonic Plague had a devastating effect on European society. It is believed to have begun in China, and it reached European soil in 1347, when it struck Constantinople (Document 1). It was carried by infected fleas that spread the disease between humans and rats. A symptom of the plague was the development of large, dark swellings called “buboes” on the victim’s lymph nodes. By the time the plague left, Europe’s population had been reduced by almost half. The devastation as a result of the plague may seem shocking, but there were several important factors that contributed to its deadliness.

Signs and Symptoms of Active Tuberculosis Mycobacterium tuberculosis (MT) is a slender, rod-shaped, aerobic bacillus which causes tuberculosis. Tuberculosis (TB) is an airborn infection which is transmitted via inhaling droplet nuclei circulating in the air. These droplets are expelled from the respiratory secretion of people who have active TB through coughing, sneezing, and talking (Porth, 2011). Some bacilli stay in the upper airway and are swept out by mucus-secreting goblet cells and cilia on the surface of the airway.
The rail market continued to grow and by the 1860’s all major cities within the United States were connected by rail. The main diseases that showed the most virulence during the time were cholera, yellow fever and consumption, now known as tuberculosis. The 9th census mortality data showed that 1 out 7 deaths from disease were caused by tuberculosis and 1 out of 24 disease deaths were resulting from cholera. . Throughout the ages, while the origins to this day continue to be debated, the strength and potency of the disease have rarely been in question. Syphilis, while not viewed as a huge threat due to a decreased number of cases in the mid-late 1990s, needs to be taken more seriously by the public because it is more dangerous than many realize, especially because it is extremely contagious, it is extremely elegant in the symptoms it produces, it has played a larger part in history than many would think, and there is a certain stigma which surrounds the disease, which in turn pushes individuals away from receiving the necessary testing.
The Bubonic Plague, or more commonly known as ‘The Black Death’ or ‘The Black Plague,’ was one of the most devastating and deadliest pandemics that humans have ever witnessed in the history of mankind. The disease spanned two continents in just a few years, marking every country between Western Europe all the way to China. During the reign of the plague, which is estimated to be the years between 1347-1352, it is estimated that “20 million people in Europe–almost one-third of the continent’s population” was killed off due to the plague. The Black Plague would change the course of European history since the plague knew no boundaries and inflicted its wrath upon the rich and the poor alike. As a result, not only did the plague have a devastating demographic impact which encountered a massive social disruption, but also, an economic and religious impact as well.
Tuberculosis has many forms that can invade the body. Many cures involve drugs and multiple combinations of the drugs. Drugs are used in combinations because tuberculosis can travel to different parts of the body. One of the causes for the spread of tuberculosis around the world is the emergence drug- resistance strain. Tuberculosis can become resistant to most, if not all, of the drugs that are used to treat tuberculosis.
Education can be a powerful weapon in fighting tuberculosis in the United States also around the world. Today, it is encouraging how so many people know how TB contracts human and what cause drug resistant effects among those who are under TB treatment.
Tuberculosis is transmitted from person to person through airborne droplets, when a person that is infected with TB coughs, sneezes, talks, and/or sings letting tiny droplet to be released into the air(Bare, Smeltzer, Hinkle, and Cheever, 2008). TB cannot be spread through touching inanimate objects, food, or drinks (Bare et al. 2008). The person must be in the same area an affected individual is in and inspirate the droplets to be affected. Once the bacillus is inspired into the lungs, the bacilli start to multiply causing lung inflammation also known as nonspecific pneumontis (Huether et al. 2008). To cause an immune response the bacilli will travel through lymphatic system and become lodged in the lymph nodes (Huether et al. 2008). Lung inflammation causes the activation of the alveolar macrophages and neutrophils (Huether et al. 2008). Granulomas, new tissue masses of live and dead bacilli, are surrounded by macrophages, which form a protective wall. They then transform into a fibrous tissue mass, the central portion is called a ghon tubercle (Bare et al. 2008). The bacterial then necrotic, forming a cheesy mass, this mass may become calcified and form a collagenous scar (Bare et al. 2008). At this point, the bacteria becomes dormant and there is no further progression of the active disease. The disease can become active again by re-infection or activation of the dormant bacteria (Bare et al. 2008).
In modern medicine when an ailment arises it can be quickly diagnosed, attributed to a precise bacteria, virus, or body system, and treated with medication, surgery or therapy. During the time before rational medical thought, this streamlined system of treatment was unheard of, and all complaints were attributed to the will of the multitude of commonly worshiped Greek gods (Greek Medicine 1). It was during the period of Greek rationalism that a perceptible change in thought was manifested in the attitudes towards treating disease. A remarkable breakthrough in medicine occurred in the late 1800s through the work of Louis Pasteur. Pasteur's experiments showed that bacteria reproduce like other living things and travel from place to place. Using the results of his findings, he developed pasteurization, which is the process of heating liquids to kill bacteria and prevent fermentation. He also produced an anthrax vaccine as well as a way to weaken the rabies virus. After studying Pasteur's work, Joseph Lister developed antisepsis, which is the process of killing disease-causing germs. In 1865 before an operation, he cleansed a leg wound first with carbolic acid, and performed the surgery with sterilized (by heat) instruments. The wound healed, and the patient survived. Prior to surgery, the patient would've needed an amputation. However, by incorporating these antiseptic procedures in all of his surgeries, he decreased postoperative deaths. The use of antiseptics eventually helped reduce bacterial infection not only in surgery but also in childbirth and in the treatment of battle wounds. Another man that made discoveries that reinforced those of Pasteur's was Robert Koch. Robert Koch isolated the germ that causes tuberculosis, identified the germ responsible for Asiatic cholera, and developed sanitary measures to prevent disease. (1)
Tuberculosis is an air-borne disease, hence, it can be passed from an infected person to a healthy individual through coughing, sneezing and other salivary secretions. Tuberculosis is caused by the transfer of Mycobacteriun Tuberculosis (M. Tuberculosis) also known as Tubercle Bacillus, a small particle of 1-5 microns in diameter, due to the small size, when an infected person sneezes or coughs, about 3,000 particles are expelled. M. Tuberculosis responsible for tuberculosis is able to stay in the air for a long period of time (about 6hoursAnother way of acquiring Tuberculosis is by drinking unpasteurized milk, milk straight from cow, although this is not a common mode of transmission, it can be found in rural areas. Ingestion of contaminated cow milk transmits Mycobacterium Bovis, the animal form which is still potent enough to cause tuberculosis in humans. ). Tuberculosis transmission is affected by exposure, socioeconomic status of person, proximity, immune status of uninfected individual (%&&%&? CDC).
Tuberculosis is transmitted by inhalation of aerosols containing the tubercle bacilli. The required inoculum size for infection is usually high, but easily occurs with exposure to a patient who is currently infected. The products of dried aerosols, droplet nuclei, are particularly infectious because they remain in the air for an extended time, and upon inhalation easily move to the alveoli. The severe damage related to infection is caused by the reaction of the host. The tuberculosis infection has two phases, primary and secondary.
Researchers have also identified a list of other factors associated with TB transmission such as environmental, organism and host factors, but these unfortunately, are outside the scope of the present paper. 16
