Approximately 5 million people living in the United States suffer from congestive heart failure (CHF) and half of those diagnosed will die within 5 years. An individual may present to the hospital with weakness, short of breath (SOB), swelling of the extremities, ascites, and breathing difficulties while lying down. The quality and length of life for someone suffering from heart failure can be improved with early diagnosis, medication, physical activity, and diet modification (CDC, 2013).

Without each systems our bodies would not function properly, but what happens when one of these system fails? The Cardiovascular system components are blood, blood vessels and the heart. The hearts function is to pump blood to all the major organs and tissues (Circulatory). It’s also important to be aware of the derivation that our bodies may encounter, for example a stroke. Congestive Heart Failure is when the heart's pumping power is weaker than normal. It does not mean the heart has stopped working. The blood moves through the heart and body at a slower rate, and pressure in the heart increases. This means; the heart cannot pump enough oxygen and nutrients to meet the body's needs. The chambers of the heart respond by stretching to hold more blood to pump through the body or by becoming more stiff and thickened. This only keeps the blood moving for a short while. The heart muscle walls weaken and are unable to pump as strongly. This makes the kidneys respond by causing the body to retain fluid and sodium. When the body builds up with fluids, it becomes congested. Many conditions can cause heart failure, and they are Coronary artery disease, Heart attack, Cardiomyopathy, and conditions that overwork the heart.

The scientific journal I selected discusses the cardiac disease, congestive heart failure. In this article registered nurses and doctors came together to talk about a new way to improve patients functioning lifestyle while battling with this awful disease. Discovering that with the new healthcare system the readmission rates of patients with congestive heart failure, there was something more they could do to improve the outcome of the medical setting in which these patients are being treated.
Nurses are fundamental to the process of implementation of certain systems used in the workplace. Nurses should be allowed to have input and suggestions regarding what works and what doesn’t when it comes to those systems, whether it is the EMR, healthcare organization systems, or even the use of point of care systems (Mitchell, 2011). This allows for open collaboration between the nurses and information technology to come up with solutions and user friendly applications when needed. As a result, patient care as a whole has positively been affected within the last decade. Furthermore, it only make sense that more personalized and precise problem-solving methods and procedures will be devised in the future. Accordingly, the following paragraphs will analyze the significance of the Meaningful Use program for nurses, nursing, national health policy, patient outcomes, and population health associated with the collection and use of the programs core criteria.

Cardiac remote patient monitoring uses smart phones, and specific designed (secured) e-mails to deliver information sent from the device implanted within the patient’s heart.
