Since I have been in the health care field for over nine years as a Radiographic technologist in an orthopedic center. I have seen many older patients receiving poor quality of care after surgery, especially from a nursing home. One incident that still plays in my mind till this day is a patient that has been seeing our doctor Mr. Gonzalez for years for chronic back pain. Mrs. Brown was a single but very social and active 62 year old women, until one day walking her dog, she slipped, fell and broke her hip. Luckily for Mrs. Brown that day doctor Gonzalez was on call that day and performed emergency surgery on her hip, because Mrs. Brown had hip replacement surgery she was admitted to a nearby skilled nursing facility to recover and receive rehab. A few weeks later after surgery Mrs. Brown was due for her postoperative appointment and no showed. Brown skilled nursing facility is the three core model process. According to Kelly, (2011) “the three core model process represents the view of a health services delivered (pp.48-62).”Three care model process is grouped into three core categories such as clinical/ medical/technical process, operational and patient flow and administrative process. This model would help managers in the case of Mrs. Brown properly implement guidelines and train staff members on how to address the postoperative care process, when dealing with patients after surgery. Three core model process would also help an organization promote a patient focused orientation by helping managers recognize the need for aligning process and improvement efforts toward the needs of the patient, this would have helped management in Mrs. Brown nursing facility schedule per-diem nursing assistance to accommodate the shortage of staff and have records on who took care of Mrs. Brown during each shift
