Pre-trajectory phase-Course of illness has not yet begun shows no signs and symptoms noted.
Trajectory phase-sign and symptom of the disease appear and a diagnostic workup starts.
Crisis phase-Life threatening situation
Acute phase-symptoms are severe and unrelieved that requires hospitalization.
Stable phase- Symptoms are under control and manageable

Being diagnosed with a chronic illness is a life-altering event. During this time, life is not only difficult for the patient, but also for their loved ones. Families must learn to cope together and to work out the best options for the patient and the rest of the family. Although it may not be fair at times, things may need to be centered on or around the patient no matter what the circumstance. (Abbott, 2003) Sacrifices may have to be made during difficult times. Many factors are involved when dealing with chronic illnesses. Coping with chronic illnesses alter many different emotions for the patients and the loved ones. Many changes occur that are very different and difficult to get used to. (Abbott, 2003) It is not easy for someone to sympathize with you when they haven’t been in the situation themselves. No matter how many books they read or people they talk to, they cannot come close to understanding.
The conceptual model guiding this paper is A Middle-Range Theory of Self-Care of Chronic Illness. The theory defines self-care as a process of maintaining health using health promoting practices and management of illness. It emphasizes that self-care should be performed in both healthy and ill states. In addition, the theory further emphasizes that it is imperative for every individual to engage in some level of self-care from day to day by making informed food choices and rational healthy living styles. Attitudes are the foundation of quality of care for older adults. Among health care professionals, discrimination and stereotypical behaviors are very prevalent, even though more often than not these individuals do not realize their actions are ageist. “Ageism hinders people from seeing the potential of aging, anticipation their own aging, and being responsive to the needs of older people” (McGuire, Klein & Shu-Li, 2008, p. 12). Attitudes are directly correlated with how individuals age and whether individuals stay health and live longer (McGuire, Klein & Shu-Li, 2008, p. 12). The care that older adults receive from healthcare professionals is directly influenced by that provider’s attitude about growing older. All too often, health care providers rely on a patient’s chronological age rather than their functional age when determining their needs and what interventions are prescribed. Another issue lies in providers viewing the complaints of older patients as a part of “normal aging”, therefore potentially missing life-threatening problems that may have been easily resolved. “Age is only appropriate in health treatment as a secondary factor in making medical decisions, and it should not be used as a stand-alone factor” (Nolan, 2011, p. 334).
Chronic illness issues can include managing their illness, the cost of taking care of the illness, etc. Many people who suffer from a chronic illness suffer a lot trying to manage their illness on a daily basis. According to a website called NCOA.org, “About 80% of older adults have one chronic disease. 68.4% of Medicare beneficiaries have two or more chronic diseases and 36.4% have four or more. Chronic diseases can affect a person’s ability to perform important activities, restricting their engagement in life and their enjoyment of family and friends”
A long-term health condition can also be referred to as a ‘chronic illness’ and it can be used to define recurring conditions such as asthma, arthritis, and diabetes.
There are many ways to categorize illness and disease; one of the most common is chronic illness.
