List the stages of the chronic illness trajectory and give a 1 sentence description of what is happening during this stage.
Pre-trajectory phase-Course of illness has not yet begun shows no signs and symptoms noted.
Trajectory phase-sign and symptom of the disease appear and a diagnostic workup starts.
Crisis phase-Life threatening situation
Acute phase-symptoms are severe and unrelieved that requires hospitalization.
Stable phase- Symptoms are under control and manageable
Unstable phase- Inability to keep the symptoms under control
Downward phase-Progressive deterioration and symptoms increases.
Dying phase- gradual shutting down of the body process
Identify 3 possible interventions to improve care for older adults in;
Acute care settings
• Targeting high risk patients
• Improve communication between health care providers treating the patient
• Encouraging family involvement and educating them about the patients illness
