The five principles of HP include building healthy public policy, creating supportive environments, strengthening community action, developing personal skills, and reorienting health services (McMurray & Clendon 2015). The first principle aims to incorporate health into all public policy decisions beyond the health system so that living and working conditions become conducive to health and equity (Germov, Freij & Richmond 2015). According to McMurray & Clendon (2015), multi-sectoral collaboration is required among different sectors, such as education, industry and social welfare, with the reciprocally influential policies that guide the community health. The second principle emphasises the socio-ecological approach to health that promotes sustainable environment and broader social support systems that encourage a safe and satisfying life (Germov, Freij & Richmond 2015; McMurray & Clendon 2015). The aim of this program is associated with the PHC principle of equitable access to health care, and health promotion and disease prevention. CEH promotes the program by working with communities and health services, which is related to the PHC principle of community participation and a multi-sectoral approach (CEH 2016a). MHSS supports the PHC principle of the use of appropriate technology by providing individuals with assistance to make appointments for screening and diagnostic tests (CEH 2016b). The program applies the HP principle of reorienting health services by operating Multicultural Sexual Health Network that gathers people across multiple sectors to share information, improve service coordination, and research ways to enhance health for refugees and migrants (CEH 2016b). The four major ethical principles in health care are: Autonomy – to honor the patient’s right to make their own decision (the opposite is paternalism - the health care provider knows best for the patient), Beneficence – to help the patient advance his/her own good, Nonmaleficence – to do no harm (many bioethical controversies involves this principle), and Justice – to be fair and treat like cases alike. All 4 principles are considered to be in effect at all times. In theory, each is of equal weight or importance. The United States’ population is currently rising exponentially and with growth comes demographic shifts. Some of the demographics shifts include the population growth of Hispanics, increase in senior citizens especially minority elderly, increase in number of residents who do not speak English, increase in foreign-born residents, population trends of people from different sexual orientation, and trends of people with disabilities (Perez & Luquis, 2009). As a public health practitioner, the only way to effectively eliminate health disparities among Americans, one must explore and embrace the demographic shifts of the United States population because differences exist among ethnic groups (Perez, 2009). We must be cognizant of the adverse health conditions for each population and the types of socioeconomic factors that affect them. Culture helps shape an individual’s health related beliefs, values, and behaviors. It is more than ethnicity and race; culture involves economic, political, religious, psychological, and biological aspects (Kleinman & Benson, 2006). All of these conditions take on an emotional tone and moral meaning for participants (Kleinman & Benson, 2006). The diversity among the U.S. population is very large and continue to grow, especially the Hispanic group. More so, health promotion can be defined or perceived in many ways depends on the minority group and their culture beliefs. As health care provider, recognizing and providing cultural competent is very important. In addition, assessing the health disparities among the minority group and teaching them how to promote good health will benefit along the way. Furthermore, health care providers have the role to promote good health but without proper education and acknowledge cultural awareness will be impossible to accomplish.
From patients not wanting people of color to deliver care to them and patients from different religion having difficulties with the value of care. Healthcare organizations are working tremendously to develop cultural competence in it facilities. An article titled “Why Diversity matters in Health Care?”, defines cultural competence as “the ability of systems to provide care to patients with diverse values, beliefs and behaviors, including the tailoring of delivery to meet patients’ social, cultural and linguistic needs.” Diversity is one important aspect in the healthcare system because different people from different backgrounds, cultures and religions all seek care one or the other. By every individual accepting and acknowledging that we are all different then there will be mutual respect for each other’s cultural difference. When these differences are explored in a safe, positive, and nurturing environment, they are more than simply tolerating others but that through these differences are instead commended and embraced. People become aware of each other’s difference and uniqueness such as religion, sexual orientation, race, gender, age and beliefs. When this is embraced by the health care providers then there will be improved care.
Healthy People 2020 is a program for the promotion of health and the prevention of diseases, launched by the Department of Health and Human Services in December 2010. According to healthypeople.gov, this program has four overarching goals which are first to achieve healthy, longer lives free of preventable diseases, injuries, and premature deaths; to achieve health fairness, eliminate differences, and improve all groups’ health; also to produce social and physical environments that encourage good health; and last but not least to promote life’s quality, healthy development, and healthy behaviors through all life stages. This program has a vision of a community where people live long, healthy lives. Healthy People 2020 offers a comprehensive set of 10 years of nationwide goals and objectives that is meant to improve the health of the American population. Healthy People 2020 covers 42 topic areas with approximately 600 objectives, which include 1,200 measures. A smaller set of Healthy People 2020 objectives, has been designated to communicate high-priority health issues and actions that can be taken to address them, this objectives are called Leading Health Indicators. The program goes above and beyond these health indicators in order to provide the best care for the people of this country. These indictors cover from the access of health service, nutrition, physical activity, and obesity to substance abuse, environmental quality, injury and violence.
