Fístula Perianal Criptoglandular
La enfermedad fistulosa constituye una de las patologías anorrectales más frecuentes y aunque existen pocos datos acerca de su verdadera prevalencia, se estima que puede significar entre el 10 al 30 % de las intervenciones coloproctológicas(1). Es conocida desde la antiguedad y ya aparece descrita en el Corpus Hippocraticum(2), que ya recoge su tratamiento mediante el uso de sedales o la puesta a planto. Ya en el siglo XIV John Arderne(3), publica un monográfico que describe tratamientos muy similares a los aplicados en la actualidad. Sin embargo, no sería hasta el siglo XIX cuando Frederick Salmon(4) fundara en 1835 “The Infirmary for the Relief of the Poor afflicted with Fistula and other Diseases of the Rectum”, germen del que sería en 1854 el “St. Mark’s Hospital for fistula and other Diseases of the Rectum” origen y estandarización de la cirugía coloproctológica en Europa y el mundo.
Respecto a la etiología de la enfermedad fistulosa, la teoría más aceptada establece su origen criptoglandular partiendo de una infección en el espacio interesfintérico(1).
Parks, radicado en este mismo hospital, establecería la clasificación de la fístula perianal basándose en la relación entre el trayecto fistuloso y el esfínter anal. Establecería por tanto la diferencia entre fístulas interesfinterianas, transesfinterianas, extraesfinterianas (transesfinterianas altas), combinadas y fístulas en herradura(5).
Utilizando en esta clasificación, el diagnóstico por un cirujano experto contituye el pilar básico en el manejo de esta patología. Estos espacios son ocupados por material purulento produciendo tanto una respuesta sistémica caracterizada por fiebre y malestar general mediada por los factores inflamatorios y por otro lado una respuesta local basada en los signos clásicos de la infección: tumor, dolor, calor y color. La historia natural finaliza con el del material purulento a través de la piel o, en ocasiones, del anodermo (puede ser un drenaje espontáneo o quirúrgico).
Tras el episodio agudo del absceso perianal pueden seguir dos situaciones: la primera es la curación, finalizando así el proceso. La segunda es que la glándula origen persista infectada desarrollando el proceso crónico de fístula perianal.
Por tanto una fístula perianal es la perpetuación de una infección glandular que debutó en forma de absceso.
