Review of Research: Everyday Excellence: A Framework for Professional Nursing Practice in Long-Term Care

A nursing home is normally the highest level of care for older adults outside of a hospital. Nursing homes provide what is called custodial care, including getting in and out of bed, and providing assistance with feeding, bathing, dressing and medication needs. However, nursing homes differ from other senior housing facilities in that they also provide a high level of medical care. Skilled nursing care is available on site, usually 24 hours a day. Other medical professionals, such as occupational or physical therapists, are also available.

The Everyday Excellence framework is based upon eight guiding principles: Valuing, Envisioning, Peopling, Securing, Learning, Empowering, Leading, and Advancing Excellence. The framework intends that RNs are empowered to enact the professional values, best practices, and caring spirit of geriatric nursing when it is integrated into and supported by the structures and processes of a long-term care organization. Registered nurses working within this professional model gained confidence in their clinical skills and leadership abilities. Nursing is constantly evolving and changing, in order to be more efficient in providing care than in the past. The nursing profession includes professionals who are not only caregivers but support systems as well as educators. All these factors help to provide optimal care for patients and to also better serve their families and the community. All nurses are encouraged to break down the simplistic notion society has about the nursing profession because nursing is a multi-faceted profession encompassing many different factors that are beneficial to overall human development and health.
Leaders recognize improving clinical quality will have many benefits for both, residents and the organization. For example, it will benefit residents by improving satisfaction, reducing complications, and improving their quality of life. It will benefit NCH by reducing turnover, improving occupancy rates, attracting top talent, and enhance employee satisfaction (Advancing Excellence in America’s Nursing Homes, n.d.). Increasing professional development opportunities will be key for improving clinical quality. Senior nursing students will complete a QSEN weekly clinical journal requirement learn how to self-assess their progress toward demonstrating these nationally-based competencies. The students will select a different competency each week to address and discuss how they applied that competency to patient care or how they hope to better achieve that competency as a graduate nurse. By the end of the clinical rotation each student will have had a chance to focus on each of the six QSEN competencies: patient centered care, teamwork and collaboration, evidence-based practice, quality improvement, safety and informatics. The students’ reflection on their clinical experiences each week will teach them how to integrate the core competencies required before graduation. The nursing career has a growing workforce, spurred on by the high demand for caregivers. The growing need for nurses has caused in influx of new nurses, graduating from school and ready to begin their career. Although there are many different work settings for nurses, one universal aspect of assisting new nurses is nursing leadership. Leaders within nursing are tasked with assisting new nurses as well as those who are veteran nurses, and their role is indispensible. “Health leaders model the behavior expected in the organization” (Ledlow & Stephens, 2018). Susan Eckert, the senior vice-president of nursing and chief nursing executive at Medstar Washington Hospital Center, is a prime example of a nursing leader.
The American Nurses Association (ANA) developed a foundation for which all nurses are expected to perform their basic duties in order to meet the needs of the society we serve. The ANA “has long been instrumental in the development of three foundational documents for professional nursing; its code of ethics, its scope and standards of practice, ands statement of social policy.” (ANA, 2010, p. 87) The ANA defined nursing as “the protection, promotion, and optimization of health and abilities, prevention of illness and injury, alleviation of suffering through the diagnosis and treatment of human response, and advocacy in the care of individuals, families, communities, and populations” and used to create the scope and standards of nursing practice. These facilities are regulated by the state and federal government and these regulations protects the senior residents. For example it is mandatory for the facilities in Texas to provide mandatory services such as daily living activities like dressing, feeding or help prepare meals and cleaning. Depending on the facility license the staff would have to assist with financial management and certain medical services. Even though the federal government developed guidelines the state can make their own as long as it complies with the federal government. Some organizations may accept private pay while others accepted Medicaid. Regulations are developed to protect residence that from being in an unsafe environment. As a result some assisted living and nursing homes are unable to continue services by having fines or closing for an unknown amount of time. Since each state has different set of regulations I will focus on the state regulations in Texas because it is the state I reside in. The organization in Texas that regulates assisted living and nursing homes is the Department of Aging and Disability services(DADS).
There are many complications that can arise as one ages. As stated in the Aging Concepts and Controversies (2012) book, there are basically two worlds of aging; the ill-derly and the well-derly. While those that are well are able to take care of themselves or need very little help to perform their daily activities; those that are ill most likely we need to have some form of long-term care. As stated earlier, when looking at care options for older adults, there are two forms of care that many people try to decide between. The first option is home care. In this form of long-term care, the elder person is looked after by a spouse, family member such as a child or grandchild, or a close family friend. The other type of long-term care is nursing home or institutionalized care, where the elderly person is taking care of by professionals and lives in the facility where they are being taken care of. I incorporate Erickson’s Modeling and Role-Modeling Theory in my daily nursing practice. The most recent advances in health care would not be possible without nurses, who strive to make continual improvements and advancements in health care. By successfully completing my journey of becoming a nurse practitioner, I am working toward better opportunities and advancements in health care. I plan to use the advancements and new developments that I learn during my nurse practitioner journey to give back to my community.
Nurses not only provide care but they play many other roles. For example, nurses are considered a communicator, teacher/educator, counselor, leader, advocate and collaborator (Taylor, Lillis, Lynn and LeMone. 2015). As a caregiver, nurses provide physical, emotional, intellectual, and spiritual needs. In addition, as a caregiver, nurse integrates the roles of communicator, teacher, counselor, leader, researcher, advocate, and collaborator to promote wellness through activities that prevent illness (Taylor et al. 2015). The primary role of a nurse is providing care to patients. One positive behavior a nurse should have is keeping boundaries between work and personal life. If a nurse is having a really bad morning at home, she shouldn’t express her emotions while she is working with patient. The patient and the coworkers don’t need to suffer because a nurse is having a bad day. Another positive quality every nurse should have is good working habits. They should come to work at least half an hour early so they can get organized and prioritize their work before starting the shift. In a health care setting, everyone works as a team. Nursing excellence is defined in various ways. I believe that nursing excellence is demonstrated when nurses are involved in promoting holistic patient centered care, quality and innovation, education, evidence-based practice, and nursing research. Nursing organizations plays a pivotal role in promoting nursing excellence. For this discussion post, I will be discussing the role of two organizations; the National League of Nursing (NLN) and the Quality and Safety Education for Nurses (QSEN), in promoting nursing excellence and nursing practice.

It is the facility’s duty to offer a sense of comfort, enjoyment, and happiness to its residents. Visiting a nursing home is the best way to find out if that is true or not, because unfortunately that is not always the case. If it can not meet these simple notions listed below, it is not a qualified nursing home to accommodate the elderly. A proper facility should include a pleasant and healthy environment, staff that is passionate about helping residents, and a working team or system that can care for the elderly.
