In the United States, community health field is anchored in the history of innovations of public health methods and programs aids at reducing risk factor prevalence, decreasing acute and chronic disease burden and injury occurrence, and promoting health (Goodman, Bunnel, Posner, 2014). “Community Health refers to the health status of a defined group of people and the actions and conditions, both private and public (governmental), to promote, protect, and preserve their health” (McKenzie et al., 2005).

Many government health agencies conducts these assessments for assuring the safety of public. Agency for toxic substances and disease registry conducts public health assessments for the purpose of determining the extent of exposure of harmful substances. The benefits of the public health assessment is that it helps in planning for the future health needs of the public by developing policy and other proper planning (CDC, 2005).
Collecting data and proper information some major steps in assessment process and so conflicts will arise if the proper data is not collected. One of the example where error occurred was a 2005 report of Turkey. This report had no information on how the researchers selected their village sample, and so as a result, its findings ' broad applicability was difficult for the assessment process. The focus of public health has been shifted according to events and particularly a community making it hard to define. However this makes it crucial to determine the situation under the recent public health, such as health needs assessment, in order for action to be taken with the economic situation, epidemiological transitions related with stigma, demographic status, health literacy and life course approach, and facilitating day-to-day activities (Wilson and Mabhala, 2013).

According to Allender, Rector, and Warner (2014), public health is a combination of both an art and a science (2014). The mission of public health nursing is to promote health, prevent disease and ultimately prolong life (Allender et al., 2014). In order for this to occur an assessment must take place. An aggregate or community assessment begins with a collection of data. This includes: the community’s health needs, risks, environmental conditions, financial resources through local census data, and a windshield survey (Allender et al., 2014). Through public health nursing, communities can collectively come together to help promote an overall better health standing.

The Community Themes and Strength Assessment (CRSA) is one of the MAPP assessment process that used to know the important things in a community, the perceived quality of life in a community, and the assets that can be used to improve the community health. This process’s goal is to understand the community health issues, concerns, quality of life, and the community assets’ map (NACCHO, 2016).
Today there are so many factors that affect the quality of health in our communities. In the project, I will assess the quality of health in my community. Throughout history the church has been a major factor and good starting point when attempting to understand the community. For this project I choose New Hope Baptist Church located at 284 Vine Street in Jackson, Ms pastured by Rev. Dock Cooper III. New Hope is located in area code 39206, and approximately 2 minutes from the Interstate 55, and 10 minutes from downtown Jackson. There are several churches located in this zip code (39206); also Highland Village shopping center is one of the more Up-scale shopping centers in Mississippi. This shopping center has over fifty stores, clothing jewelry, sporting and pharmacy. Schools are also spread out over this area Elementary, junior high and high schools. Jackson Academy is also located in the area, this school is widely known as one of the finest schools in Mississippi. Most of the homes and companies are old and are currently under re-construction. The population in the area is very high comprised of mostly middle-aged and young adults. The Jackson Police Department’s precinct #4 is located west of New Hope Baptist Church. Union Planters bank and Trust mark bank and a branch of the post office located in the center of the community. This community is well known as being in, “The Middle of everything”. And finally this is the community I have lived in the past 8 years.
Public health can be defined as the approach to medicine that is concerned with the health of the community as a whole. It is about protecting and improving the health of families and communities through promotion of healthy lifestyles, research for disease, injury prevention, detection, and control of infectious diseases. Many factors contribute to a long life, including demographics, socioeconomics, genetics, environment, and behaviors. It is important to assess these factors when attempting to increase life expectancy and improve health status, because positive health practices result in higher life expectancies and better health.
The purpose of this community profile is to discuss a particular health improvement issue within a chosen community. A community can be defined using many different terms. You can belong to a community through religious beliefs. Through shared experiences or interests. You can belong to a political community where all involved share the same political interests. A community can also be defined as ‘a family’ a small village where many have lived most of their lives who share the same desire to belong to that community.
This is a community profile that aims to identify a specific health improvement issue within a local geographically determined community. ‘A community profile is an attempt to describe a particular community or neighbourhood. It uses a variety of different techniques to build up a picture of the community from a number of perspectives’ (Barnardos.org.uk, 2004) including several components of a community such as its demographic characteristics, patterns and trends i.e. its epidemiology in order to make comparisons between different localities to determine areas needful of specific health improvements with the goal of improving local people’s health and reducing health inequalities. A core definition of community as distinguished by Macqueen and Mclellan et al (2001) is ‘a group of people with diverse characteristics who are linked by social ties, share common perspectives, and engage in joint action in geographical locations or settings’.

Assessment is a tool that helps monitors different health and environmental statutes to create, deploy, and identify solutions. It also used a diagnostic tool to investigate health-related problems and different health hazards. An idea of a healthy community with a safe environment, accessibility to preventive health care services and screenings, education on health promotion that is stress-free and accessible and healthier grocery stores. Characteristics of a healthy community are ideas, activities, and resolutions derived from partnerships of diverse groups, regular self-evaluation that is part of the practice, which is the focus. A diversity of strategies, techniques, documents, and other resources would have to be executed to support and make a healthy community.
