I. INTRODUCTION
As one of the primary health care providers in the medical team, pharmacists have the opportunity to influence the patients’ perception in their medications. The pharmacists played an important role in the health care system. The availability of and rational use of medicines are critical for a successful therapeutic outcome. Though rapid developments in science and technology have led to easy understanding of etiology and pathophysiology basis of various diseases and development of new molecules, many times clinicians fail to achieve the desired therapeutic goals. Items on the questionnaire were grouped in three categories by type of subject matter as follows: Attitude to professional responsibilities, pharmacist attitudes towards reasons for deciding to counsel, and pharmacist barriers toward reasons against deciding not to counsel. All items were scored on the frequency of approval to the assessment of each subject matter. Collected data were then analyzed and results are presented as means and standard deviations. This paper discusses pharmacology and terminology related to “Pharmacology” which is the branch of medicine concerned with the uses, effects, and modes of action of drugs“ pharmacology. 2015. In Merriam-Webster.com. The study of different classes of drugs, routes of absorption, and drugs have effects on those consuming them. There are drugs that are necessary for illnesses and healing but, there are medicines that cause concern regarding interaction and harming the body.
The Consulting Pharmacist: Role is to support the use of non - pharmacologic approaches as initial treatment plan for the management of behavioral and psychological symptoms of the patients, indicating an appropriate use for medications, specific and documented goal of therapy; ongoing monitoring, and also ensuring that medication use does not exceed the required time/duration of treatment.
Studies have shown that patients fail to adhere and comply with medicines because of lack of understanding. [1] Gordon et al (2007) conducted a study which aimed to identify medication related problems and identify approaches to support patients to manage their medication. Problems with access to and the organisation of services was one of the findings highlighted from the study. Pharmacy can address the issues associated with organisation of services, patient education and engaging patients in decision making about their medicines in the home during consultation.
In conclusion, physician-patient relationship is important element in educating patients about the importance of medication adherence. Age, gender, cost, and side effects are few of the reasons that influence compliance but none are as crucial and important as strong patient-physician relationship. Highlighting advantages/ disadvantages of treatment, formulating a detailed regimen to implement the treatment, making frequent follow ups are very crucial to developing a strong patient-physician relationship and lowering the chance of any illness or diseases. Although both physician and patient are responsible for a successful treatment, there is a greater weight on the patient to make sure their treatment is running successfully.
Any related health event to expand clinical pharmacy services and to reflect the role of pharmacists in health care team. Also , emphasize the important role of pharmacist in influencing the thinking of decision-making individuals. On the other hand advocacy can be applied by patient - empowerment, in which pharmacists give information to their patients and encourage them to ask to help them take the right decision. However, Current and pharmacist future must participate in all activities where advocacy can be applied, in order to promote health and wellbeing of society .
Since my early schooling days, science and mathematics have always fascinated me given that all components of the world are explained on the basis of these disciplines. In this regard, Pharmacy which is based on Chemistry and Mathematics has interested me a great deal as it has direct impacts on our daily lives. The more I have become familiar with the chemistry and mathematics knowledge involved in pharmacy, the more I have appreciated its significance in improving the lives of people in the community. Out of the many careers that one would opt to pursue, Pharmacy has stood out as the career that will enable me serve the society more directly and have a real impact in the quality of peoples’ lives by touching their health. In addition, I love a career that is challenging and one that I will be involved in even in old age. In this case, Pharmacy offers me this opportunity. My interest in Pharmacy has also developed from my personal health experiences and becoming aware of its expansiveness. Every time I have had to take medicine to cure an illness, I have become more interested to know how drugs are made and how the drug components work in the human body to alleviate pain and restore health. I desire to pursue a career in Pharmacy so as to gain practical experience in these aspects of research.
My personal interest in the pharmaceutical profession rooted for my desire to be involved in the improvement of my family’s and other people’s health. My mother who suffered from asthma and often had shortness of breath. She can’t exercise or stay outdoor for a long period of time especially during the colder weathers. As a young child, I have to witnessed my mother suffer from breathing problems through out the years. This made me feel helpless and I wished I could do more to help her. The only factor that improved her health is medication. Today, I am thankful to see that her asthma condition improves due to the help of medications. After witnessing all the troubles that she went through, I am inspired to become a pharmacist where I can applied my medicine knowledge to improve not only my family’s but also other’s health.
As a way to prepare pharmacy students for real world practice, one of the assignments done in the P1 year is to perform a videotaped patient counseling session. Over the past few weeks prior to the counseling session we had various lectures and lab actives to help us prepare for patient counseling. In the following paragraphs, I will describe what I feel are my strongest abilities, my areas of weakness, and what I plan on doing to help improve these areas of weakness.
I pleased to apply to the PharmD program as the program is one area that corresponds to my career dreams. Being part of this program gives one the opportunity to gain an excellent experience in working and collaborating with various health care providers in the ward. But more importantly, it facilitates a practical environment in dealing more closely with patients. Hence, it helps to provide the ultimate health care services to patients. Also, it permits me to carry on gaining different knowledge, skills, and values in addition to those I have already developed during my undergraduate studies. My interest in being a clinical pharmacist was first aroused during my SPEP rotation in the hospital setting where I was really impressed with the role of clinical pharmacists who provide a consistent process of patient care with healthcare teams to maintain the appropriateness, effectiveness and safety of the medication use. Unlike a pharmacist, a clinical pharmacist has a more diversified responsibilities and closeness to direct patient care. Moreover, provides

First, here is some important background information about Pharmacists. A Pharmacist is someone who is trained and licensed to distribute medicinal drugs and to advise on their use. Patient counseling may be defined as an interaction between a pharmacist and a patient. It is a one to one interactive session, which should include the teach-back method, which is a way to ensure patient understanding. Patient counseling is a very fundamental course, especially for a pharmacy student. As a healthcare professional, counseling patients is very essential in order for patients to adequately take their medications properly, reduce adverse side effects and prevent drug interactions.
The world of pharmacy has always been prevalent in my life due to the fact that my mother is a community pharmacist. My enthusiasm for the field began during childhood when my mother would convince me to take medication by explaining the positive affect it would have on my body. Due to my interest in my mother’s job, understanding the mechanisms of how medications treated ailments captivated me and inspired me to learn more about her occupation. As a result, I often shadowed my mother at the local pharmacy and observed her duties as a pharmacist. The most memorable part was watching her counsel her patients. I can still recall my amazement at the knowledge she had acquired over her years of practice. Though the job seemed stressful at times,
Pharmacists often work together in a team with other healthcare professional like physicians and nurses. In the process, pharmacists will give advice to them on the selection of medication, by providing the evidences based on the dosage form, the side effects and possible interaction with food of the medication. On the other hand, pharmacists also take part in research and clinical studies. Recently, pharmacists are recruited to conduct pharmacy-based research in pharmacies. (Swanson, 2005)
