At the onset of assessment by a staff-counseling psychologist, the woman seemed to relax and share some of her thoughts and feeling. As the assessment process continued, the psychologist was able to ascertain that the issue with depression appeared to be a relevantly recent development. Additionally, the depression appeared to be the result of heighten conflicts between the woman and her husband pertaining to alleged extra material affairs. In conversation with the psychologist, the woman claimed to feel “overwhelmed”; her husband filing for divorce triggered the feelings she inferred, which reportedly lead to her breakdown. However, the psychologist has since discovered that the husband denies the affairs and attributes this to the depression. Additionally, the husband claims that the termination of the marriage is a result of the deterioration of his wife’s mental state.
The ethical discernment model described by Slosar (2004) and developed for use at Ascension Health will assist us as we analyze this case. It reminds us that discernment engages our spirituality, intellect, imagination, intuition, and beliefs. It is decision-making that reaches into the heart of our beliefs about God, creation, others, and ourselves. It therefore requires structured time for reflection and prayer from the beginning and throughout the process.
First, the therapist attempts to investigate the behaviours that the client presented on the first time that she experiences the problem. Second, the therapist tries to understand the way the client is managing her symptoms and problems (Dobson and Dobson, 2009) by identifying the safety behaviours that the client is adopting to reduce the level of anxiety (Papworth, Marrinan, and Martin, 2013). On the video session, the therapist showed concern about the behaviours that the client was engaging on (Marshall and Turnbull, 1996), however, she should have asked her more about specific behaviours that the client was probably engaging on, based on the information that the client provided (Kinsella and Garland, 2008). The therapist tries to detect behaviours such as avoiding specific situations, like for example leaving the house alone (Papworth, Marrinan, and Martin, 2013), yet she did not explore this enough. The therapist should have also inquired the client about reassurance seeking and safety seeking behaviours, as the client stated that she calls her husband when she is feeling anxious. The counselor was professional in giving feedbacks pertain to his or her potential clients. The therapist present a realistic approach based on the couple situation and facts. I let my client tell me how he felt about illness based on his own values and beliefs. I also used therapeutic communication techniques such as general lead, listening, sitting at eye level with the client etc. to make this as comfortable as possible for him. I think I was appropriate and very successful at retrieving my client’s beliefs about illness and sickness without pressuring him too much. In the future when conducting an interview with another person about their culture, I would use the same techniques and approach as I found it to be very successful, and my client was very comfortable and established a trust worthy relationship with him regarding his illness based on his cultural beliefs and values. This interview contributed in preparing me for the future and also gave me insight on how to conduct a successful cultural assessment without making assumptions. I learned that every culture is unique and has somewhat of a similarity to other cultures, but one must never assume before doing a thorough assessment.
