Recently, a frail, elderly woman was admitted to the medical unit with a diagnosis of aspiration pneumonia. This would be her third hospitalization in a short period of time for the same diagnosis. A mild stroke several years ago left her with swallowing issues. She was not in good underlying health with a number of comorbidities. She required additional care at home and her closest relatives were adult children who lived in another state. Widowed for several years; she was lonely and had survived many of her longtime friends. She was quite ill when she arrived on the unit with an elevated white count and abnormal blood gases. She required a large facial mask to deliver oxygen via BiPAP. She was assessed to be oriented to person, place and time on admission. At that time it was also determined her code status to be a “Full Code”. Intravenous antibiotics and fluids were started. With her underlying Congestive Heart Failure (CHF) it was evident she would need to be monitored closely for fluid overload. The ANA code of ethics “is the definitive framework for ethical and decision making for RN’s across all practice levels, roles and settings” (ANA Code of Ethics, 2016). The nurse in the scenario above is driven by provisions one through four in the code which addresses respect and dignity for the patient, a primary commitment to the patient’s interests when there is not an agreement with clinicians or family, advocacy for the patient and an obligation to provide the best possible care. Being familiar with the nursing code of ethics, what is ethically and morally expected in society and how to approach the situations can help make dilemmas less of a nightmare. “The purpose of nursing ethics is to inspire questions and examine what would be the ethically right action in health care situations demanding a choice between at least two undesirable alternatives” (Toren & Wagner, 2010, p. 394). Through centuries nurses were given the title “Caregivers”. Unlike some doctors, nurses actually care for their patients, not necessarily saying doctors do not; they both just have a different way of caring. Yes, doctors cure illnesses, but nurses are just as important because they help with the healing process. Most nurses can have the same exact education or knowledge as a medical physician but the only thing individuals see is a name tag with either the acronym CNA, LPN, R.N. and PH.D. Of course PH.D will get all the credit, seeing as how nurses do not exactly diagnose patients. A nurse could just become a doctor but there are different aspects of each title. Nurses take instructions from a higher administrator, which is sometimes a doctor. What needs to be known is doctors are not the only ones that stress and have rules to abide by. Nurses have ethical codes, daily ethical dilemmas, morals, and ridiculous distress, but some of these examples differ with country, state, and hospital. If nurses are capable, then they should be given the opportunity to make medical decisions or diagnosis in critical situations.

Ethics and integrity are essential parts of the nursing profession since they provide nurses with the capacity for weighing in on the impacts that their actions may have on the profession (Guido, 2014). “Ethics is a systematic study of principles of right and wrong conduct” (Taylor, Lillis, Lynn, & LeMone, 2015, p. 96). The American Nurses Association Code of Ethics has nine provisions to it. All nine are important to the nursing field in their own way. There are two provisions that I find most important in helping my career as a nurse. Another huge ethical topic is the patient’s right to choose autonomy in the refusal of life-saving medicine or treatment. This issue affects a nurse’s standards of care and code of ethics. “The nurse owes the patient a duty of care and must act in accordance with this duty at all times, by respecting and supporting the patient’s right to accept or decline treatment” (Volinsky). In order for a patient to be able make these types of decisions they must first be deemed competent. While the choice of patient’s to refuse life-saving treatment may go against nursing ethical codes and beliefs to attempt and coerce them to get treatment is trespass and would conclude in legal action. “….then refusal of these interventions may be regarded as inappropriate, but in the case of a patient with capacity, the patient must have the ultimate authority to decide” (Volinsky). While my values of the worth of life and importance of action may be different than others, as a nurse I have to learn to set that aside and follow all codes of ethics whether I have a dilemma with them or not. Sometimes with ethics there is no right or wrong, but as a nurse we have to figure out where to draw the line in some cases.
The nursing code of ethics has a very standard definition. It is the base on how nurses should guide themselves in conduct by making the right decision regarding ethical issues. According to the National Student Nurses Association “students of nursing have a responsibility to society in learning the academic theory and clinical skills needed to provide nursing care” (2003). In the clinical setting nurses have a lot of responsibilities while caring for an ill patient, they have the obligation to practice their profession with compassion, love, and respect the uniqueness of each patient, as nurses we are not supposed to deny care to a patient because of their economic status, their skin color, race, or the nature of health problems, we are here to help the people in need in particular those of susceptible populations. The NSNA states that the code of conduct is based on an understanding that to practice nursing as a student is an agreement that trust and honesty is depended on us by society. The announcement of the code provided direction for the nursing student in the personal development of an ethical foundation and not limited to the academic or clinical environment but can assist in the holistic development of a person. Ethics is defined as moral principles that govern a persons or a group’s behavior, ethical principles apply to both personal and professional relationships (Webster, 2015). The field of nursing is a profession that has been highly regarded and respected in society. Most nurses enter the profession in order to utilize their clinical skills to help others in their time of need. Those in failing health rely on nurses to care for them in their most vulnerable states, and expect a level of compassion and humanity while receiving care. The American Nurses Association Code of Ethics for Nurses has five elements that pertain to the Principle of Autonomy. Each individual element applies to “respect individual persons” (Baillie, McGeehan, Garrett T, M., Garrett R. M., 2013, p.33). In Chapter 2 of the Health Care ethics: Principles and problems text, it discusses thouroghly the consent of an individual to make their own decisions regarding their health and future requests of care. As a nurse or within all heath care professions, we must treat each individual patient with care, respect, and to remain mindful to the patient regarding any aspect of their lives. In the ANA Code of Ethics for Nurses, it explains ways of maintaining the empathy required in the health field. It further discusses that the respect for human dignity must be a priority, relationships to patients must remain neutral, the severity of the situation, the right to self-rule, and the professionalism that must be upheld by the nurse and their associates.
