Diabetes mellitus is a collection of common metabolic disorders. The scenario of passing large amount of urine is described by the Greek and Roman physicians as diabetes whereas the term mellitus refers to sweet taste (Barrett, Barman, Boitano, & Brooks, 2012). The name of the disease reveals one of the important clinical manifestation, that is, passing sweet-tasted urine, and in the other word, the presence of sugar in the urine. Besides that, Funk (2010) stated that there are three most common symptoms in diabetes mellitus, which are polyuria (large volume of urine), polydipsia (excessive thrist), and polyphagia (excessive eating). Lecthuman et al. (2010) revealed that overall prevalence of diabetes mellitus in Malaysia, during year 2006 was 11.6 percent. Among the three races, the Indians show the highest prevalence of 19.9 percent followed by Malays 11.9 percent and lastly the Chinese with 11.4 percent. Diabetes mellitus can be discussed in several aspects, such as classification, complications , and treatment and management.
There are two types of diabetes mellitus particularly type 1 and type 2 (refer to Figure 1). Abbas, Aster, Fausto and Kumar (2010) stated that type 1 diabetes mellitus is an autoimmune disease whereby destruction of islet is primarily because of immune effector cells reacting against endogenous B-cell antigens. Besides that, Imagawa, Hanafusa, Miyagawa, and Matsuzawa (2000) reported that type 1 diabetes can be divided further as autoimmune (type 1A) and idiopathic ( type 1B), and this idiopathic diabetes have a nonautoimmune, fulminant disease categorized by nonappearance of insulin and of diabetes-related antibodies. Diabetes mellitus, habitually referred to as diabetes is caused by a decline in insulin secretion by the cells of the pancreatic islet resulting into a surge in blood glucose concentration, a condition known as hyperglycemia. Diabetes insipidus is a disorder defined by the secretion of huge quantities of highly diluted urine, this is regardless if a reduction in fluid intake. This is as a result of a deficit of anti-diuretic hormone (ADH) also known as vasopressin produced by the posterior pituitary gland. The disorder diabetes mellitus is associated by extreme reduction in weight, a higher urge for urination, also known as polyuria, higher levels of thirst (polydipsia) and an extreme craving to eat known as polyphagia. This disorder has been categorized as Type 1 or insulin dependent diabetes and Type 2 or non-insulin dependent diabetes another type is the Gestational diabetes. The Type 1 diabetes mellitus is portrayed by a deficiency of the insulin-secreting beta cells of the islets of Langerhans found in the pancreas, this leads to a shortage of insulin. The principal cause for this deficiency of beta cells is a T-cell mediated autoimmune onslaught. In children, Type1 diabetes is known as juvenile diabetes. The Type 2 diabetes mellitus is as a result of insulin resistance or diminished insulin sensitivity coupled to a reduction in insulin production.
Diabetes Mellitus is a disease of the endocrine system primarily differentiated between type 1 and type 2. Type 1 diabetes occurs when the pancreas is unable to produce insulin and was previously seen in the younger generation which is no longer the case.1 Type 2 diabetes is the more prevalent of the two types and involves elevated blood sugar levels due to the insufficient production of insulin. Risk factors that make an individual higher risk for type 2 diabetes include increasing age, obesity, family history, a sedentary lifestyle.1,2 Innovative drug therapies for type 2 diabetes remain important for the treatment and reduction of the disease.

Diabetes is a disease that affects the body’s ability to produce or respond to insulin, a hormone that allows blood glucose (blood sugar) to enter the cells of the body and be used for energy. Diabetes falls into two main categories: type 1, or juvenile diabetes, which usually occurs during childhood or adolescence, and type 2, or adult-onset diabetes, the most common form of the disease, usually occurring after age 40. Type 1 results from the body’s immune system attacking the insulin-producing cells in the pancreas. The onset of juvenile diabetes is much higher in the winter than in the summer. This association has been repeatedly confirmed in diabetes research. Type 2 is characterized by “insulin resistance,” or an inability of the cells to use insulin, sometimes accompanied by a deficiency in insulin production. There is also sometimes a third type of diabetes considered. It is gestational diabetes, which occurs when the body is not able to properly use insulin during pregnancy. Type 2 diabetes encompasses nine out of 10 diabetic cases. Diabetes is the fifth-deadliest disease in the United States, and it has no cure. The total annual economic cost of diabetes in 2002 was estimated to be $132 billion, or one out of every 10 health care dollars spent in the United States. Diabetes risk factors can fall into three major categories: family history, obesity, and impaired glucose tolerance. Minority groups and elderly are at the greatest risk of developing diabetes.
Current theories link the cause of diabetes, singly or in combination, to genetic, autoimmune, viral, and environmental factors (obesity, stress). Regardless of its cause, diabetes is primarily a disorder of glucose metabolism related to absent or insufficient insulin supplies and/or poor utilization of the insulin that is available. Pancreatic Beta cells are the only cells that regulate blood glucose. Patients with this type of diabetes usually need several insulin injections a day or and insulin pump to maintain or survive. Family history, Autoimmune disease, where the body mistakenly attacks the insulin producing cells, and environmental factors are risks factors of type 1 diabetes.
Diabetes mellitus is a group of metabolic disorders characterized by inadequate insulin secretion by the pancreas or cellular destruction leading to an insulin deficiency. Depending on the cause of the insulin shortage, diabetes can be subcategorized into type I and type II. Type I diabetes (T1DM) is usually mediated by the destruction of b-cells in the pancreas resulting in decreased insulin production and secretion. Type II diabetes (T2DM) is the failure of these b-cells to secrete adequate amounts of insulin to compensate for insulin resistance and increased gluconeogenesis combined with an overall resistance to the insulin action (8., 1997). T2DM accounts for 90 to 95 percent of all diabetes cases.
Type 1 diabetes mellitus is an autoimmune disease that results in insulin dependence due to failure of secretion of the hormone insulin by beta cells in the pancreas. Type 1 diabetes usually manifests in childhood, < 20 years of age – the reason for which it was formerly termed juvenile diabetes, and is more prevalent in Caucasians then in any other ethnicity (Diabetes Basics, n.d.). The key to proper management is early detection, patient education, and most importantly patient compliance.
An astounding 347 million individuals (WHO-Danaei.G) are inflicted with Diabetes mellitus (DM), a type of metabolic disease that’s significantly influenced by hyperglycemia (causing individuals to generate high blood sugar) and can be characterized by the insufficient production or improper reaction to insulin. There are three different types of DM: Type 1, Type 2, and Gestational diabetes, all of which share common symptoms include polyuria (frequent urination), polydipsia (increased thirst), and polyphagia (increased hunger) (Ahuja 1139). Generally, diabetes prevents the ingested glucose from entering the cells, ultimately depriving the body of its’ main source of energy.

It is often referred to by doctors as diabetes mellitus and described as, “… a metabolic disease in which the person has high blood sugar …” (Collazo- Clavell et all. 2009), either because the insulin is inadequate or the body’s cells don’t respond well to the insulin. The health and economic consequences of diabetes are considerable. The majority of people that have diabetes live in low and middle income countries, where the prevalence of the disease is high. There are three types of diabetes that are called type 1, type 2, and gestational diabetes. Although diabetes is a disorder, it can lead to other diseases such as heart attack, kidney failure or death. A person that has diabetes has to maintain a healthy lifestyle, by eating the right diet, controlling their blood sugar level, and be optimistic.
Pre-diabetes is a condition in which blood glucose levels are higher than normal but aren’t high enough to be considered diabetes (Kishore, 2014). Type 1 diabetes is formally known as insulin-dependent diabetes or juvenile diabetes. In type 1 diabetes, the body’s immune system acts against the insulin producing cells of the pancreas, and over 90% of those cells are destroyed permanently (Kishore, 2014). The pancreas is left with no choice but to produce a very small amount of insulin or none at all. For this reason, sugar is not able to reach into the body’s cells to be used for energy (Cleveland Clinic, 2013). Of all the people living with diabetes, only about ten percent live with Type 1 diabetes (Kishore, 2014). Type 2 diabetes is formally known as non-insulin dependent diabetes or adult-onset diabetes. In this type, the pancreas sometimes produce insulin at higher levels than normal. Unfortunately, the body then develops a resistance against the effects of insulin. Diabetes has been around for 2000 years. Over the years there has been a vast improvement in treatments which are still ongoing to this day. In the 17th century, diagnoses were done through urine tests and one was diagnosed if one’s urine was sweet. In the 20th century, doctors would advise patients to go on radical diets of less than 450 calories per day. In 1921 insulin was finally discovered.

Insulin is a hormone that helps the body’s tissues absorb glucose which is sugar, so it can be used as a source of energy. Glucose levels build up in the blood and urine which causes excessive urination, thirst, hunger, and problems with fat and protein metabolism in a diabetic person. Diabetes is very common in the United States; it is the seventh leading cause of all deaths. Women have been diagnosed with diabetes more than men. There are two forms of diabetes, Type one and Type two diabetes. Type one diabetes is when the body does not produce insulin or produces it in very small quantities. This usually occurs in younger people under twenty years of age, mostly around puberty. Type two diabetes is when the body’s balance between insulin production and the ability of cells to use insulin doesn’t work properly. This is more common than type one; about 90-95% people in the United States have it. There are no cures for diabetes now but there are many researchers investigating factors through new technologies to cure them. Meanwhile, technological advancements are being made to keep glucose at a good level for diabetes.
