An effective Electronic Medical Record (EMR) is more than a management tool for controlling patient data. Methods used to monitor implementation, relationship between organization’s processes, professional roles, and communications techniques used to address issues during implementation are points to develop in this paper. For years now, the healthcare system in the United States have managed patient’s health records through paper charting, this has since changed for the better with the introduction of an electronic medical record (EMR) system. This type of system has helped healthcare providers, hospitals and other ambulatory institutions extract data from a patient’s chart to help expedite clinical diagnosis and providing necessary care. Although this form of technology shows great promise, studies have shown that this system is just a foundation to the next evolution of health technology. The transformation of EMR to electronic heath record system (EHR) is the ultimate goal of the federal government.
With an average of more than three million instances of child abuse reported annually in the United States, social workers face an overwhelming client list of children and adults who are or have been victims of cruelty and negligence. Left untreated, the chances that these individuals may lead lives fraught with future psychological conditions are increased exponentially. Since many children and some adults are incapable of verbalizing disturbing experiences, a treatment called Eye Movement Desensitization and Reprocessing (EMDR) can often provide the resolution that more traditional therapies can’t. Overall EMDR therapy is important because it helps to process distressing memories, reducing their lingering effects which allow patients to develop more adaptive coping mechanisms and lead normal lives.
“Meaningful Use” implemented in July, 2010, set criteria’s for physicians and hospitals to adhere, in order to qualify for certain financial incentives and to be deemed meaningful users (MU) of the EMR. Meaningful use in healthcare is defined as using certified electronic health record to improve quality, safety, efficiency, and reduce mortality and morbidity. There are 3 stages of meaningful use implementation. The requirements for the 3 stages are spread out over a period of 5 years. MU mandates that physicians meet 15 core objectives and hospitals meet 14 core objectives (Hoffman & Pudgurski, 2011). The goal is to in-cooperate the patient and family in their health, empower autonomy to make decisions while improving care in all population.

This was made possible by the creation of the electronic health record (EHR), electronic medical record (EMR) and personal health record (PHR). The EHR, PHR and the EMR allowed for patient’s paper medical charts into transformed into electronic charts. This allowed for a better way to organize the information that was contained the paper medical chart. The health organization began to realize they could use these electronic charts for a better way to care and share patient health related information. However, as the transmission of data continued, the need for standards developed to insure the interoperability of these healthcare systems. Two of the standards that were created in order to help with the electronic transmission of medical data are the Continuity of Care Record (CCR) and the Continuity of Care Document (CCD).
This paper will identify the use of Electronic Health Records and how nursing plays an important role. Emerging in the early 2000’s, utilizing Electronic Health Records have quickly become a part of normal practice. An EHR could help prevent dangerous medical mistakes, decrease in medical costs, and an overall improvement in medical care. Patients are often taking multiple medications, forget to mention important procedures/diagnoses to providers, and at times fail to follow up with providers. Maintaining an EHR could help tack data, identify patients who are due for preventative screenings and visits, monitor VS, & improve overall quality of care in a practice. The impact the EHR could have on a general population is invaluable; therefore, it needs special attention from a trained professional.

The EHR, started in the 1960s under the name of "computerized-based patient record" (CPR), became known as "electronic medical records" (EMR) in the 1990s and today it is known as electronic health record (EHR).The target of the Department of Health and Human Services (HHS) is to incorporate the EHR and use it in a "meaningful" way to improve the quality, efficiency, and safety of patient care delivery; to engage patients in their personal health record; and to improve care coordination. The purpose of this paper is to discuss how Electronic Medical Records (EMR), affects healthcare delivery. I will discuss the positives and negatives this issue has on healthcare and how it effects the cost and quality for healthcare services. In addition, I will identify any potential trade-offs to cost or quality. Lastly, I will discuss how the EMR affects my job as well as any challenges or opportunities this issue presents.
