This scenario presents the various challenges that a typical nurse at the bedside of a client would be required to resolve by assessing, referring and co-ordinate the necessary resources pulled from the various health disciplines from the doctor, paramedic and social etc. to sustain vital functioning, decrease discomfort and improve health over the long term. Thus, it would be necessary to carefully evaluate the situation to determine the next course of action since time is of the essence in acute situations like this as a gatekeeper to client’s health. I would begin helping my client by first asking for his or her name, current location and situation in which he or she is in at the moment of calling. This would be necessary to ascertain whether the client is in a safe environment and also know the exact location so as to dispatch emergency services to a safe location be it necessary during the course of my tele- health intervention as client’s symptoms worsens. Second, I would like to know how long the client had experienced this problem, is he or she experiencing any pain and if present should describe it’s characteristics such as it’s location, whether sharp or dull in nature and also point out any blue discoloration in the lips or fingers. Hence, by asking this question, I am building a set of objective benchmarks to first determine whether these symptoms are chronic or acute in nature and also assess the functioning of her cardio-pulmonary system. This helps me to decide whether I need to call the emergency dispatch services based on the answers that I am provided with whereby cyanosis or a pain of 10 would warrant such an action. Third, I would like to know what kind of water pill my client is taking and... ... middle of paper ... ...ilability of the necessary health care providers such as home health nurse, physician and home aide. Additionally, a chaplain could be also contacted to help client deal with personal problems if the reasons of non-compliance to his or regimen where due to physical and emotional stresses of living. In a nut shell, it would require the patience of the tele-health nurse to carefully listen to the client’s concern and ask the appropriate questions in order to determine the right course of action needed since recovery of health by the client would require the timely decision of the nurse to know when it’s the appropriate time to refer the client the next health expert. Thus, the nurse has to fulfill his/her fundamental duty of being the gatekeeper that determines, organizes and co-ordinates care for the client using the various health professionals available.
