Healthy People 2020 is geared towards betterment of health for the most citizens by year 2020.It is a carefully and exhaustively planned project by the different sectors of government and health agencies with the inclusion of the analyses of the determinants of health, and disparities, using different kind of public health models in order to make appropriate policies and programs that are feasible to implement and practical to abide for the benefit of the population served.
Objectives & Brief Content Outline:
With the knowledge that overall health cannot be substantially improved and disparities reduced without addressing the social determinants of health, this paper aims to (1) Define Healthy People 2020 and identify the social determinants they measure on individual, interpersonal and community level. (2) Identify public health models that HP 2020 use and the policies that are currently in place. (3) Describe the overall relationship of these public health models, policies and social determinants to Healthy People 2020.
Defining Healthy People 2020 and the health determinants they measure:
Healthy people 2020 can be defined as that arm of the government launched by the Department of Health and Human Services in December 2010 with 4 goals which include attaining high-quality, longer lives free of preventable disease, disability, injury, and premature death; achieving health equity, eliminating disparities, and improving the health of all groups of people; creating social and physical environments that promote good health for all people; and promoting the quality of life, healthy development, and healthy behaviors across all stages of life (CDC, 2015).

Determinants of health fall under several broad categories: Policymaking, Social factors, Health services, Individual behavior and Biology and genetics. It is the interrelationships among these factors that determine individual and population health. Because of these, interventions that target multiple determinants of health are most likely to be effective. Determinants of health reach beyond the boundaries of traditional health care and public health sectors; sectors such as education, housing, transportation, agriculture, and environment can be important allies in improving population health. Policies at the local, state, and federal level affect individual and population health. .Some policies affect entire populations over extended periods of time while simultaneously helping to change individual behavior.
