MATERIAL AND METHODS This prospective randomized controlled clinical trial was conducted from September 2012 to October 2013 and included patients who underwent microincision cataract surgery (MICS) at the Hospital Virgen de los Lirios. The study recruited 60 eyes of 41 patients. Eligible patients were those aged 65 to 80 with senile cataract and no other concomitant disease that would prevent a postoperative best corrected visual acuity (BCVA) of 20/40 or better. Exclusion criteria included history of ocular surgery or trauma, corneal disease, glaucoma, uveitis, vitreous opacities, retinopathy or visual pathway defects. Other exclusion criteria were current treatment with systemic steroids, immunosuppressants, anticoagulants or prostaglandin analogue eye drops. Patients with intraoperative complications or extended surgical time were also excluded. After informed consent was obtained, participants with history of allergy to penicillin were assigned to the vancomycin group (Group 1). An equal number of patients were selected at random and assigned to the control group (Group 2). Information gathered from patients included date of birth, sex, history of drug allergy and systemic diseases, medication use and surgeon. Patients in Group 1 received an intracameral injection of vancomycin (1 mg in 0.1 ml), whereas those in Group 2 received an intracameral injection of cefuroxime (1 mg in 0.1 ml). Vancomycin injection was prepared from the commercially available vancomycin hydrochloride 500 mg powder (Combino Pharm SL, Barcelona, Spain) and diluted with balanced salt solution (BSS®, Alcon Cusí, Barcelona, Spain) to a 1 mg/0.1ml solution. Statistical analysis Data were encoded and analyzed using the statistical package SPSS Statistics (version 21.0, SPSS Inc, Chicago, USA). Comparability between groups for baseline demographic and clinical characteristics was ensured. Changes in UCVA, refraction, BCVA, corneal pachymetry, IOP and endothelial cell count between preoperative and postoperative examinations were calculated using paired t test. Comparisons between groups were performed using unpaired t test for continuous variables and Mann-Whitney U test for ordinal variables. A value of P<.05 was determined to be significant. The results are given as the mean ± SD.

Cost and ECP recommendation would likely be the major factors a patient would consider when choosing type of disposable lens. This was evidenced by the differences in adoption between the two trials (p13). Other considerations would likely include travel frequency, activity level and ocular symptoms (p10, 13 & 22).

It was good to hear that you had a beneficial time at your last clinical site. The rehab unit you described definitely had all the makings of a clinical microsystem. “A clinical microsystem can be defined as the combination of a small group of people who work together on a regular basis to provide care and the subpopulation of patients who receive that care” (Nelson, Batalden, & Godfrey, 2007, p. 233). Every microsystem has its ups and downs, but hopefully the good always outweighs the bad. The clinical site I attended also had a little trouble when explaining medication to patients. It really depended on the nurse because some did a fabulous job explaining meds and some completely forgot to. Luckily we both know that explaining medications
