The goal for nurses as a profession is not only to be “patient advocates” but also assist the patient to learn and gain the necessary skills to achieve the best level of functioning for the patient based on their current illness. In order to help a patient achieve their optimal level of functioning the nurse must work with the patient and the interdisciplinary team to create a collaborative plan that is logical for the patient. Through examining a musculoskeletal disorder case study #35 from Preusser (2008), one can create a critical pathway for the patient, S.P. a 75 year old female, with severe rheumatoid arthritis (RA) and admitted to the orthopedic ward for a hip fracture status post fall (p. 183). Since the patient’s needs is unique and complex the nurse must tailor a plan with the patient which will include “…assessments, consultations, treatments, lifestyle changes, disease education…” in order for the patient have the most appropriate evidence-based care and make informed decisions when it is necessary (Oliver, 2006, p. 28). The aim for the nurse caring for the S.P. is to help prepare the patient for an upcoming procedure and focus care to the patient by gathering necessary information about her while. Collaboration with the patient, family members, rehabilitation, medical and surgical team about the treatment plans can help us provide proper patient’s care by utilizing actions and interventions within the scope and standards of the nursing practice.

The general idea of, K, is that a nurse must have knowledge in the diversity of cultures, ethics, and education. Patients often have complex care needs, and often present with multiple co-morbidities or problems. The process of conducting a comprehensive nursing assessment, and the coordination of care based on these findings is central to the role of the Registered Nurse (NMBA 2006). Evidence-based interventions must then be planned and implemented in a patient-centred approach in order to achieve agreed treatment goals and optimise health (Brown & Edwards 2012).
A long term condition not only brings the physical symptom of pain, but a number of psychological and social effects too. In 2014 the Department of Health recognised that the impact of having a long term condition can contribute to mental health problems like depression and anxiety (Department of Health, 2014). As well as finding ways to manage their physical symptoms, patients are encouraged to adopt acknowledge and address all of their health and wellbeing needs, in particular self-management at home and incorporating and educating the patient’s family and close friends as a support system (Kraaimaat and Evers, 2003). The suffering that a person with chronic pain endures not only impacts on their life, but also affects their family, time lost from employment and uses up precious healthcare resources.
Arthritis affects people of all age groups. More than 100 types of arthritis are known. Among these osteoarthritis and rheumatoid arthritis having the highest incidence. One of the major causes of chronic debilitation in industrialized nations is Osteoarthritis which results from damage to the joints, which may be due to trauma, infection, or age-related wear. Rheumatoid arthritis is encountered less frequently than osteoarthritis and is estimated to affect around one per cent of the world’s population. Amongst patients of Rheumatoid arthritis, women are three times more likely to be affected by this condition than men. This condition is caused by an inflammatory process where the body starts attacking itself. Rheumatoid arthritis also affects several joints, with inflammation sometimes seen in and around the lungs, the heart, the eyes and the skin. The most commonly reported complaint by arthritis patients is pain. The pain might be from the joint itself and be a result of inflammation, damage from the disease, or through daily wear and tear. Muscle pain is also common and is caused by having to force movements against stiff and painful joints. Although range of movement in the affected joint may be limited and uncomfortable, physical exercise has been shown to benefit those with arthritis. Physical therapy has been shown to significantly improve function, decrease pain in the long term and delay the need for surgery in advanced cases. The majority of arthritis cases occur among theelderly, however the disease can occur in children as well. Throughout this program, this author has learned the various ways critical thinking is utilized in nursing. This applies to developing care maps, Kaplan testing, using the nursing process, medication administration, and patient care. Kaplan testing has been the hardest aspect of thinking critically for this author. However, with practice and remediation, this author has learned how to critically think through the questions in order to choose the best possible answer. In correlation, this author now understands that thinking critically leads to effective, high-quality care and patient outcomes by understanding the needs of patients and treating those patients with evidence-based care.

Around the 1960s, nursing educational leaders wanted to formulate a nursing theory that contained knowledge and basic principles to guide future nurses’ in their practice (Thorne, 2010, p.64). Thus, Jacqueline Fawcett introduced the metaparadigm of nursing. Metaparadigm “identifies the concepts central to the discipline without relating them to the assumptions of a particular world view” (MacIntyre & Mcdonald, 2014). Fawcett’s metaparadigm of nursing included concepts of person, environment, health, and nursing that were interrelated. The metaparadigm ultimately contributed to conceptual framework to guide nurses to perform critical thinking and the nursing process in everyday experiences in clinical settings.
During my residency, I had the opportunity to work with the Rheumatology Rehabilitation team at the University of Alberta (U of A). I met passionate mentors in physical and occupational therapy dedicated to helping arthritis patients preserve and optimize joint function. There was one man with rheumatoid arthritis attending the program who said the most important learning point he wanted me to take away was how much his rheumatologist and the rehab team had changed his life. Prior to starting biologic therapy, he was completely debilitated in his day-to-day life and unable to perform simple activities such as picking up his young children. He now found himself not only with disease control, but also quality of life. I value the multidisciplinary approach in rheumatology and emphasis on treating the “whole” patient, rather than a single organ
