Analyzing an Ethical Decision
Health care providers, especially nurses, encounter circumstances in which they are expected to simultaneously act in the best interest of patients, families, other colleagues, and the health care organization (Mennella, 2015). For Advanced nurse practitioners (APRNs) the intricacy of the issues intensifies because nurses are considered as moral agents. Hence, APRNs are expected to be leaders in identifying and resolving moral problems, as well as creating conducive ethical practice environments, and promoting social justice in the health care systems (Hamric, Hanson, Tracy, & O’Grady, 2014). Because the pain described in the case is one that has been witnessed by the nurses that has directly taken care of the patient and performing the daily dressing changes. The dressing changes had always been sources of excruciating pain for the patient in her early stages of her diseases process when she was able to show her pain cues. The nurses felt the moral distress for not providing relieve of pain during the patient dressing change. The role of the nurse according to American Nurses Association code of ethics Provision 3 “The nurse promotes, advocates for, and protects the rights, health, and safety of the patient” (ANA, 2015). The duty of the nurse is to take action to protect patients and to ensure that the impaired individual receives proper assistance
Nurses everywhere face problems and challenges in practice. Most of the challenges occur due to a struggle with the use of ethical principles in patient care. Ethical principles are “basic and obvious moral truths that guide deliberation and action,” (Burkhardt, Nathaniel, 2014). Ethical principles that are used in nursing practice include autonomy, beneficence, non-maleficence, veracity, confidentiality, justice, and fidelity. These challenges not only affect them, but the quality of care they provide as well. According to the article, some of the most frequently occurring and most stressful ethical issues were protecting patient rights, autonomy and informed consent to treatment, staffing problems, advanced care planning, and surrogate decision making (Ulrich et. al, 2013). The ethical issue of inadequate staffing conflicts with the principle of non-maleficence.
The first provision of the American Nurses Association’s (ANA) “Code of Ethics” states, “ The nurse, in all professional relationships, practices with compassion and respect for the inherent dignity, worth and uniqueness of every individual, unrestricted by considerations of social or economic status, personal attributes, or the nature of health problems.” The second provision states, “The nurse’s primary commitment is the patient, whether the patient is an individual, family, group, or community” (Fowler, 2010). A registered nurse (RN) is someone that went through a university or college and studied nursing; and then passed the national licensing exam to obtain a license to practice nursing. In the medical profession, doctors and nurses run into ethical dilemmas every day whether it be a mother who wants to abort her baby or a patient who has decided they want to stop cancer treatment. It is important for the nurse to know where they stand with their own moral code, but to make sure they are not being biased when educating the patient. Nurses are patient advocates, it is in the job description, so although the nurse may not agree with the patient on their decisions, the nurse to needs to advocate for the patient regardless.
During week 4, we became familiar with the application of ethics in the nursing practice settings. We learned about ethical theories and principles, which are crucial when practicing in any clinical settings during ethical decision-making and while facing one or multiple ethical dilemmas. “We can’t turn away from a patient’s pain just because it’s difficult” (chapman, 2015, p. 88). I know the path of least resistance is taking a path of ignorance. Easy, is to ignore or neglect the true pain patients experience in times of crisis. As caregivers I believe we all want to heal others or we wouldn’t be in the field of nursing, but there are only handfuls willing to be present during the healing process because sharing one’s pain hurts. As a surgical nurse, I find being genuinely present takes hard work on my behalf, not only mentally but emotionally. On a unit where patient’s needs and conditions are changing at astonishing rates, being present requires mental strength in order slow down enough to recognize the value presence
There are many who believe that the next shortage will be worse and the demand for nurses will increase. There will be more jobs available especially with the baby boomer nurses retiring. Wood believes that when nurses retire, the next shortage could be even worse than the previous shortage. According to Wood this would lead to an “intellectual drain of institutional and professional nursing knowledge” (Wood, 2011, para 15). Staiger agrees as well that a shortage of nurses is expected again when nurses retire and since the economy will be more stable full-time nurses will go back to being part-time (Huston, 2017). The concept of moral distress can be defined in a number of different ways. Generally speaking, when individuals make moral decisions about the right course of action in a situation but are unable to carry it out, they will experience moral distress. A man by the name of Andrew Jameton defined moral distress in 1984 as “a phenomenon that arises when one knows the right thing to do, but institutional constraints make it nearly impossible to pursue the right course of action” (Nursing Forum, 2007). Because of their particular position in the medical world and their conflicting responsibilities, nurses are especially prone to experiencing moral distress. Whether they are aware of it or not, nurses are becoming more and more involved in making ethical decisions regarding their patients. However, the doctor’s policy always rules over the nurse and the patient’s wishes must always be respected. The consequences of not being heard by fellow co-workers or having your plan of action overruled by other policies can often be frustrating and upsetting for a nurse. While there is no data that directly connects the effects of moral distress and the quality of care nurses give, it can be inferred that the feelings of moral outrage, frustration, and anger cause nurses to care for patients in a less effective way. A nurse who is at conflict with him/herself and those that surround him/her will experience difficulties in treating a patient with the best care.
The term nursing ethics means that the nurse has an obligation judge what is right and wrong in her or his duties as guided by the profession or the moral principles that govern the profession and as prescribed by the professional body. Nursing ethics initially encompassed virtues that were desired in a nurse. At the time, these virtues included physician loyalty, commitment to high moral character and obedience. Evolvement of nursing profession gradually made nurses embrace patients’ advocacy. As patient advocates, nurses work as part of an interdisciplinary team to provide patient care. Nursing ethics have kept pace with the advancement of the nursing profession to include a patient-centered focus, rather than a physician-centered focus.
In critical and complicating medical cases, family members often find it tedious to decide as to what mode or procedure of treatment is idyllic for the recovery of their patient. In such cases, well-qualified and medically educated can play a pivotal role in deciding the kind of treatment that should be given to the patient to enhance its recovery. In a contrary situation a nurse may know that administering a particular drug may improve the patient’s condition, but may be refrained from conducting the required action due to doctor’s absence or non-permission. Integrity, respect for persons, justice, non-maleficence, and responsibility are all identified within the code, however compassion is not directly stated but is implied. When nurses decide to act either beneficently or non-maleficently they are doing a service to those being cared for. When dealing with human lives moral value becomes especially important, and is not situationally dependent. The code deals with specific issues related to the nursing profession, and ensures standards are upheld.
