Although given specific subtypes and indicators, within these symptoms lies a “zone of ambiguity” which may lead to an increase of diagnosis (Parens and Johnston, 2009). The issue of the DSM’s attempt to label those is the issue of severity. It attempts to give a diagnostic label to different children with various levels of severity and different collections of symptoms. Because children’s behaviors may be sporadic and unpredictable, a haziness appears within what levels of symptomatic behavior are clinically significant versus what levels are normal in the typical adolescents. Consequently, many parents after reading a DSM description of symptoms are apt to conclude their child has ADHD. “ADHD: Attention-Deficit Hyperactivity Disorder in Children, Adolescents, and Adults” claims that most attributes of ADHD are not unusual and many of these symptoms are present in all children at some particular times. The signs are only abnormal when they are excessive and to a severe degree (Wender, 2001). As previously stated, hyperactivity is a key symptom in the diagnostic process of ADHD. However, many adults can agree that children have increased energy and are continuously active. Part of development is that children actively explore their environment. It is difficult to determine what level of activity is excessive considering many children are different, therefore making it difficult to prove whether hyperactivity in a specific child is a result from ADHD. Furthermore, impulsivity is also a common characteristic of a child without a diagnosis of ADHD. It is challenging to specify how much planning and judgment one should expect from a child. Prevalence rates worldwide range from as low as 1% to as high as 20% (Parens and Johnston, 2009). Many claim that other countries in Europe are underdiagnosed due to misunderstanding of parents and underestimation of child’s abilities. In contrary, many European counties may believe that the US is overly diagnosing due to an overestimation of symptomatic behavior. That is “different environments will be more or less tolerant of active, distractible children, and will be more or less prone to see impairment from those behaviors” (Parens and Johnston, 2009). Overall, the primary care of ADHD is associated with parent’s perception of a child’s behavior as well as a sociocultural standpoint.
