As part of the educational reimbursement program being offered by ABC Bagley Company, the following bilateral contract is made effective as of September 28, 2016, by ABC Bagley Company. This contract is between ABC Bagley Company and Lulu Loveless., in which both parties (Employer & Employee) mutually agree as follows: 1. Overview of Services. ABC Bagley Company and its subsidiaries and affiliates (collectively, “ABC Bagley” or the “Company”) believes it is important to provide all employees with professional development and education. Other Considerations. Employees should understand that the Company, in its sole discretion, may modify, amend or terminate this Program at any time and that employees will be required to adhere to any such amendments. Employees are solely responsible for reviewing the current version of this Program periodically to keep abreast of all current requirements. 7. Reimbursement Procedures. Employees requesting reimbursement must complete the Tuition Assistance Program Authorization Form. Reimbursement is repaid as part of the employee’s paycheck and appears on the check detail as “tuition.” The reimbursement amount will not have any taxes taken from it nor will it be reported as income on the IRS W2 form. By participating in the Education Reimbursement Program, I Lulu Loveless understands and agrees to the following: * Must be a full-time regular employee in good standing and maintain a satisfactory performance level. * The course(s) must be related to my work. * I must receive pre-approval from my managers prior to registration for anticipated coursework.
