System Elements Documentation
Documentation in any situation is important however, documentation in system development is imperative. Most often a very important, and most times overlooked, task is documenting the internal design of software for the purpose of future maintenance and enhancement. A project plan should identify the type and level of documentation personnel must produce during each step or phase of the project. Organizations should maintain detailed documentation for each system in production. Good documentation enhances an organizations ability to understand functionality, security, and control features and may improve its ability to use and maintain the system. Documentation should include detailed descriptions, programming documentation, and operating instructions (FFIEC 1996).
There are two basic types of documentation, system and user documentation. Systems documentation includes information needed for the on-going maintenance and operation of the computer system.
Documentation is key in defending oneself, especially when cases are brought up, as my classmates mentioned, many years after the fact. Documentation is also important in order to provide a flow or a routine in the care being provided. For example, Nagelhout’s book talks about the flow of anesthesia practice during induction, in almost all

this will cause healthcare providers with the training and education needed for clinical documentation improvements to be installed effectively. It is important that having a specialized team who can create solutions towards Clinical Documentation Improvement (CDI) in order to minimize the failures that may occur. In this case, investing in training for the materials/tools necessary for healthcare providers to excel in their work with CDI. Essentially, Clinical Documentation is used throughout the healthcare system for the analysis of care, communication, and medical records. This is important because the information of medical records that healthcare providers are able to access, will help patients track their health conditions. Thus, clinical documentation improvement has a direct impact on patients by providing quality information. On the other hand, the new technological advancements will also be able to address the efficiencies in health care system that differ from paper-based charting. Improving on the quality of information will also have the effect upon the ethical and effectiveness of care that is being provided. This has a significant impact in order to maintain patient care that ensures the documentation is accurate, timely, and reflect within the services provided. Documentation assessments can be utilized so there can be improvements on the education for healthcare providers as they intend implementation standards take effect immediately. In this case, failures must be analyzed so that they will have the ability to comprehend and determine an organization’s strengths and weaknesses
Documentation is proper recording as regards to time, place, circumstances and attribution. It’s a written record of information. Communication is the first important step to documentation because nurses can’t document a patient information and medical history without interacting with them. Even the simplest statement could end up becoming very important when determining a treatment or even diagnosis.

Information Retrieval (IR) is to represent, retrieve from storage and organise the information. The information should be easily access. User will be more interested with easy access information. Information retrieval process is the skills of searching for documents, for information within documents and for metadata about documents, as well as that of searching relational databases and the World Wide Web. According to (Shing Ping Tucker, 2008), E-commerce is rapidly a growing segment in the internet.
1.0 IntroductionIn this report I will be concentrating on the failure of software systems. To understand why software systems fail we need to understand what are software systems. Software systems are a type of information system. This is because a software system is basically a means for hardware to process information. Flynn’s definition of an information system is:"An information system provides procedures to record and make available information, concerning part of an organization, to assist organization-related activities."Humans have been processing information manually for thousands of years, but with the vast increase of demand for knowledge this century has meant that a new method of information processing has been needed. Software systems have provided a new means that is much faster and efficient.
Siebel would like to close on a successful sale of their SFA system to Q&R/FleetBoston as their sales figures for the quarter pretty much depend on this one sale to Q&R. It has to convince the executive board at the company about the superiority of it products over its close competitors in order to achieve this goal.
While developing a software many complex activities are required which in turn have dependencies along them. Large software projects require the involvement of programmers, documentation specialists, program managers, architects, database analysts, system analysts, , and trainers and last but not the least testers. Along with their independent responsibilities these persons are also responsible to maintain and keep a check on the software system quality as well.
Good documentation remains in line with the NMC Code of Professional Conduct 2008 and to promote better communication (NMC 2008).
A documentation framework is essential for any large project; hence, RUP describes how to document functionality, constraints, design decisions and business requirements. Use Cases and Scenarios, are examples of artifacts prescribed by the process and have been found to be very effective at both capturing functional requirements and providing coherent threads throughout the development and deployment of the system.
