Introduction: Health outcomes refer to the changes in the health status of individuals or the population. The outcomes are attributed to multiple or planned interventions, whether or not the intention of the intervention was to alter the health status. These interventions include health services and programs including health promotion programs, government policies, laws and regulations, and consequent programs. Intervention may also include unintended or intended health outcomes of government policies in areas besides health. Health outcomes are evaluated by health indicators (World Health Organization, 1998). Health indicators are a single measure, represented quantitatively, that encapsulates an important aspect of health, such as the amount people suffering from a chronic disease. It also captures a variety of health determinants such as income, or the important aspects of the health care system, such as the proportion of patients who revisit the hospital for additional care following previous treatment (World Health Organization, 1998). These indicators can be used to describe a public health concern at a specific point in time. It can indicate periodic changes over time at the population or individual health level, describe differences in the population health, and examine the extent at which program objectives are being met. These indicators can possibly encompass illness or disease measurements which are commonly used in measuring health outcomes, such as health expectancy, life skills, and quality of life, and behaviors or actions taken by individuals related to health (Rigby & Kohler, 2002). In addition to the programs aimed at crisis intervention and suicide prevention, the MYCS offers transitional living arrangements for children and families in times of crisis. In addition they have facilities to assist with food and basic hygiene products in times of need. The Montgomery County Juvenile Court runs the Nicholas Residential Treatment Facility. This facility is a non-secure 24-bed unit which houses males, who have been adjudicated delinquent, and aims to rehabilitate these young men to become productive, responsible citizens. This mission is pursued with comprehensive treatment strategies, including family therapy and social-skill building. While staying at the Nicholas Residential Treatment Facility, youths will participate in educational, work, and recreational programs aimed at ensuring their swift and successful reintroduction to society.
One of the fasting growing juvenile treatment and interventions programs are known as teen courts. Teen courts serve as an alternative juvenile justice, to young offenders. Non-violent, and mostly first time offenders are sentenced by their peers’ in teen courts. Teen courts also serve as juvenile justice diversion programs. Teen courts vary from state to state, and sometimes within the same state. With this program, all parties of the judicial setting are juveniles with the exception of the judge. Each teen court, is designed specifically to meet the needs of the community it serves. Teen courts were created to re-educate offenders throughout the judicial process, create a program with sanctions that will allow the youth not to have a juvenile record, and to also instil a sense of responsibility.

The reason for this program is to use innovative approaches in developing programs, which will teach youth alternatives to violence, substance abuse, anger issues, low self-esteem, and the consequences as a result of making bad choices. During, the program non-violence, team building, leadership development and communication skills is promoted. A neutral and positive setting is provided for young people, so they are able to come together from different gangs throughout the city to work on their issues and programs together. When a youth is referred to the program they have an Intake-Assessment with the family. “Together, a Program Contract/Service Plan will be developed based on the individual needs of the youth/family. If the youth is on probation/parole all requirements of the courts will be incorporated into their contract. Services offered are case management, mediation, innovative focus groups, presentations, collaboration with other youth service organizations, and attendance at court hearings” (Stop ABQ Gangs,
Low self esteem, poor-decision making, dysfunctional families, poor communication skills, and associations with negative peer groups are all characteristics of delinquent youth (Journal, 1993). The Ohio Cooperative Extension Service Juvenile Diversion programs were designed to address these characteristics in juveniles. The family unit and peer association are the primary social influence on juvenile behavior and the Ohio Extension program focuses on educating families and placing juvenile offenders in programs with positive peer influences.

Juvenile delinquency is a relatively new phenomenon. For this reason, society’s reactions and solutions to the problem of delinquency are also modern developments. The United States developed the first youth court in 1899 and is now home to many new and formerly untested methods of juvenile rehabilitation and correction. One of many unique programs within the Juvenile Justice system, boot camps are institutions designed to keep delinquent juveniles out of traditional incarceration facilities and still provide a structured method of punishment and rehabilitation. Boot camps developed in the early 1990s and quickly proliferated throughout the nation. Specifically, they are “…short-term residential programs modeled after military basic training facilities” (Meade & Steiner, 2010). Designed with the goal of reducing recidivism and preventing violent offenses, boot camps target non-violent individuals under the age of 18 and typically exclude already violent offenders. In theory, boot camps apprehend juveniles while they are committing minor delinquency and prevent more-serious crime by “giving the juvenile offender a more optimistic, community oriented outlook” (Ravenell, 2002). Fundamentally, boot camps have four central purposes; rehabilitation, punishment, deterrence, and cost control (Muscar, 2008).

In order to give children a positive start to life health visitors can offer advice and support by using the Healthy Child Programme (HCP) which can take place within a home or community environment. This programme offers families a range of services including screening, developmental reviews and support on parenting and making healthy decisions. Health visitors have the skills to identify children who may not be meeting their developmental milestones and refer them onto other services to meet their needs. Healthy childhood development is key for this community because 16% of the neighbourhoods population is considered to be a child between the ages of 0-14 years (City of Toronto, 2011a). Healthy childhood development is influenced by other social determinants of health like housing, proper nutrition, and an adequate guardian income. The adjustment from incarceration to society causes a series of problems, making rehabilitation difficult. When the juvenile’s leave home to be detained, all ties with society, the support systems they had, the gangs they associated with, school they attending are no longer in close proximity, which is essential for successful rehabilitation (James, Stams, Asscher, Katrien De Roo & van der Laan 2012). Another problem association with the reintegration is that juveniles are in a particularly fragile state in that they are not only transitioning from society to detention, but from adolescence to adulthood, both of which are overwhelming adjustments. Many Juveniles have been deprived of their proper treatment due to society’s lack of understanding and compassion, yet research clearly shows that mental health treatment not only keeps them at bay from repeating their crimes, but also helps them live a more positive lifestyle in society. In times we blame the juvenile for their mistakes, however instead of pointing fingers at them, we can come together as community to help them overcome their “inner demons”. After all, it is not the children committing the crimes, but their mental disorder that is hindering them from living a normal lifestyle.
Introduction: Recidivism or, habitual relapses into crime, has time and time again proven to be an issue among delinquents, which thereby increases the overall juvenile prison population. This issue has become more prevalent than what we realize. Unless a unit for measuring a juvenile’s risk of recidivism is enacted and used to determine a system to promote effective prevention, than the juvenile prison population will continue to increase. Our court system should not only focus on punishing the said juvenile but also enforce a program or policy that will allow for prevention of recidivism. So the question remains, how can recidivism in the juvenile prison population be prevented so that it is no longer the central cause for increased juvenile delinquency? Simply put, we must create a means of measuring juvenile’s level of risk and in turn, form an effective rehabilitation program that will decrease their risk level for future recidivism.
In so doing it has created awareness and led to a discussion about the implications of social inequalities on health outcomes of individuals over the years. The theory also provides a broad perspective about disease processes. This has allowed health researcher/professionals to design holistic treatment/care plans that does not only focus on the biomedical disease process but to take other aspects of people’s life into consideration (WHO, 2010).

Healthy People 2020 is a program for the promotion of health and the prevention of diseases, launched by the Department of Health and Human Services in December 2010. According to healthypeople.gov, this program has four overarching goals which are first to achieve healthy, longer lives free of preventable diseases, injuries, and premature deaths; to achieve health fairness, eliminate differences, and improve all groups’ health; also to produce social and physical environments that encourage good health; and last but not least to promote life’s quality, healthy development, and healthy behaviors through all life stages. This program has a vision of a community where people live long, healthy lives. Healthy People 2020 offers a comprehensive set of 10 years of nationwide goals and objectives that is meant to improve the health of the American population. Healthy People 2020 covers 42 topic areas with approximately 600 objectives, which include 1,200 measures. A smaller set of Healthy People 2020 objectives, has been designated to communicate high-priority health issues and actions that can be taken to address them, this objectives are called Leading Health Indicators. The program goes above and beyond these health indicators in order to provide the best care for the people of this country. These indictors cover from the access of health service, nutrition, physical activity, and obesity to substance abuse, environmental quality, injury and violence.
