Flight History and Incident Scenario…
On the 9th of Sept 2007 the Scandinavian Airlines Bombardier Dash 8-Q400, aircraft (LN-RDK), flight 1209, had a domestic flight from Copenhagen Airport (EKCH) to Aalborg Airport (EKYT). Up to approaching phase, it was a normal flight with no mentioned incidents, however during the approach to EKYT airport, and when the flight crew selected the landing gear down, the nose landing gear and the left main landing gear (MLG) indicated down and locked while the right MLG indicated “Transit” (not down and locked i.e. Unsafe condition to land). So the flight crew decided to perform a “go-around” to verify this abnormal condition. During the go-around, the flight crew revise the Quick Reference Handbook (QRH) for a checklist concerning an unsafe landing gear, but they did not find any checklist with that header, and the only suitable checklist -as they thought- was “ALTERNATE LANDING GEAR EXTINTION”, and it was about performing an alternate landing gear extension… So they did, but nothing changed. The flight crew subsequently tried to make a normal gear up, the nose and left MLG were retracted normally, while the right MLG indication remained in “Transit”. A second attempt to use the alternate landing gear extension was performed, but without any changes to the right MLG indication. After this point the flight crew declared the need for an emergency landing, so they inform the airport about the situation and then the aircraft entered in a holding pattern in order to reduce the amount of fuel, and to acquaint the passengers about the emergency situation. Before landing, the cabin crew decided to evacuate the seats at rows 6, 7, and 8 (the seats in parallel with the right engine propellers area). [Hcl.dk, 2007]
During the landing, the left MLG touched down the runway first, followed by the right MLG, and approximately two seconds after touchdown the right MLG collapsed, the propeller blades of the -running- right engine struck the ground, then a blade splintered from the engine and wedged in the fuselage, the aircraft then veered to the right and came to rest away from the runway. Fortunately the passengers and the crew were evacuated safely with a total of seven minor injuries amongst the four crew and 69 passengers on board. Although the flight was scheduled to depart from JFK Airport at about 7:00 p.m., it was delayed due to a disabled piece of ground equipment and concerns about a suspected passenger mismatch with baggage. The airplane took off at 8:18 p.m., shortly at 8:25 p.m., Boston air route traffic control center (ARTCC) instructed the pilots to climb and maintain an altitude of 19,000 feet and then lower down to 15,000 feet. However, at 8:26 p.m., Boston ARTCC amended TWA flight 800's altitude clearance, advising the pilots to maintain an altitude of 13,000 feet. At 8:29 p.m., the captain stated, "Look at that crazy fuel flow indicator there on number four... see that?" One minute later Boston ARTCC advised them to climb and maintain 15,000 feet to which the pilot replied: “Climb thrust”. After an extremely loud and quick sound, the cockpit voice recorder stopped recording at 8:31 p.m. At that moment, the crew of an Eastwind Airlines Boeing 737 flying nearby reported an explosion in the sky. One was a short film focusing on the 1911 Triangle Shirtwaist Fire and the second film was on the Movement for Alta Garcia. The overall purpose of this presentation was to show and raise awareness to viewers. Working conditions are extremely punitive and must be put to an end. American Labor Unions and labor laws have also drastically changed overtime.
On 24th November 2001, Crossair flight 3597 departed in the dark from runway 26L from Berlin – Tegel Airport at 20:01 UTC to Zurich Airport. 24 out of 33 people on board are killed. During the flight, the visibility was low due to overcast with light snowfall. It was further reported by the previous aircraft that the runway can only be seen from 1.3 miles. However, the commander did not find it serious, and continued with the approach.
According to an FAA spokeswoman, initial reports indicated that the plane’s engines were damaged following a double bird strike. While the plane captain declared an emergency after experiencing the bird strike, the passengers on board reported of a smell of fuel, engine flames and loss of power before the landing (Steenhor par, 4). The co-pilot, Jeffrey Skiles, who was at the controls when the flight took off, was the first to observe the formation of birds which were approaching the aircraft. Furthermore, investigators discovered that faulty switch attributed electrical actuation for the door to open. This means that the cargo door had a design flawed, which allow the door open during flight. Despite the nine fatalities, United Airlines flight 811 had landed Honolulu with 328 passengers. With that in mind, the survivals of Flight 811 was grateful for the flight crew landed safely with their cripple plane.
The flight had come in from Van Nuys Airport (VNY), Van Nuys, California. Witnesses recall that the plane had landed and parked at the FBO to receive some fuel. The plane had sat on the ramp for approximately 45 minutes before it took off again. While on the ramp, numerous witnesses recalled snow falling and “contaminating” the wings of the planes. Another theory is that the pilot, Jason Dahl might have purposefully crashed the plane to prevent the hijackers from taking it over. Or perhaps that Dahl had cut off the planes fuel with out the hijackers being aware. It was said that the plane looked like it went straight down; and that the plane had almost completely disintegrated on impact leaving a hole several feet deep.
Fifteen years have passed since American Airlines flight 1420 experienced a botched landing tragically killing 10 passengers, the captain, and injuring 110 others. Thankfully, 24 passengers were uninjured, and the first officer survived. This horrific accident could have turned out much worse, but it could have also been easily avoided.
