I have spent quite some time examining the state policies for Residential Care Facilities and assisted living facilities. These policies cover everything that one might identify within an RCF. There are regulations that dictate the evacuation of a facility. Within Preferred Family Healthcare the RCF is on the second floor of the facility. The general rule of thumb there is that individuals need to be able to navigate the stairs, moving from the second floor to the first floor and out of the building within two minutes. The state policies also state that there is a minimum number of fire drills that the facility needs to conduct within a year. This details a living room/social area, a kitchen or family style eating area, laundry accessible to the residents, a bathroom within the resident's room and they are to have an outdoor activity area. I was very surprised to see that the state regulations had specific ideas about the temperatures within the residents rooms. These regulations state that the rooms be no colder than 68 degrees and that the rooms have the ability to be heated to at least 80 degrees. The regulations state that residents rooms be "neat, orderly and cleaned daily". I had no idea that it would be so clearly and simply stated in the regulations. I felt that this was something that was implemented by staff. Another thing that I found interesting in the regulations was the statement that residents need be "free from body odor and mouth odor". I found it very interesting that the policies stated things like the fact that residents need to be provided with comfortable chairs, curtains in their room and that the hot water that they have access to have the temperature
Staff nurses in many medical settings such as Skilled Nursing Facilities are at the forefront of patient care. Many patients in these particular settings are typically suffering from some type of cognitive impairment often related to dementia syndrome, behavioral disturbances or prior mental health conditions. Many mental health symptoms are managed by second generation antipsychotics. This class of medication placed the patients at risk for metabolic syndrome.

According to policy and procedure of Volunteers of America and Veteran Affairs to assure health and safety in all residential facilities, the VOA staff will conduct 3 fire drills per month (one on each shift) and will be properly documented. Internal audits are conducted by Board and Quality Assurance staff which includes a review of facility emergency testing. These facilities are regulated by the state and federal government and these regulations protects the senior residents. For example it is mandatory for the facilities in Texas to provide mandatory services such as daily living activities like dressing, feeding or help prepare meals and cleaning. Depending on the facility license the staff would have to assist with financial management and certain medical services. Even though the federal government developed guidelines the state can make their own as long as it complies with the federal government. Some organizations may accept private pay while others accepted Medicaid. Regulations are developed to protect residence that from being in an unsafe environment. As a result some assisted living and nursing homes are unable to continue services by having fines or closing for an unknown amount of time. Since each state has different set of regulations I will focus on the state regulations in Texas because it is the state I reside in. The organization in Texas that regulates assisted living and nursing homes is the Department of Aging and Disability services(DADS).
Though this checklist is not all encompassing it provides simple guidelines a person can easily look to in order to put his/her mind more at ease and feel secure in placing a loved one in a nursing home facility. Because there is no guarantee against elder abuse occurring in nursing homes it is important that the resident, where possible, and the family of the resident remain involved on a daily basis and demand the quality of care the elder person deserves.
