It was a pleasant day at the Dementia unit and I was assigned to care for a lady whom we can call Miss K. She is a lovely, warm and nice lady who was recently admitted to the Dementia unit two weeks ago. I went to her room to meet her, I introduced myself, greeted her and gave her a hand shake for me to establish rapport. She was very cooperative, participative and independent with her cares. From my observation, I noticed that she is very organised and very particular when it comes to cleanliness. She also wakes up early in the morning and makes sure that all of the things that she needs for morning care are neatly arrange in the bathroom. When having a shower, she cleans her body very well. She also brushes her teeth and cleans her dentures very well. She always likes to wear red coloured clothes and red floral nighties. Before leaving her room, she makes sure that her glasses are clean before wearing it, her used clothes are neatly kept in the empty drawer and she wants to have a handkerchief in her pocket. To be able to give culturally safe care to her, I always follow her routine, I make sure that her preferences are met and I give high regard to it as well as her values and beliefs because she used to be a nurse and I know that those things are part of who she is, a part of her daily life and this what makes her whole. Alzheimer’s is a form of dementia that affects the brain. There is no cure for it but many treatments. Alzheimer’s is fatal and there are few stages of dementia. It is the 6th leading cause of death, more than 5 million Americans have it, and 15,5 million caregivers gave around 17.7 billion hours of unpaid care that cost around $220 billion in 2013. In my family, my grandmother who is 86 years old has very early stages of Alzheimer’s. She started having symptoms when she was 81. My grandfather took most of the care of her but as he got ill my aunt Kathy took over. When my grandfather got ill and had to be in 24 hour care, we all agree to put my grandmother in assisted living care 10 minutes from one of my aunt’s house.
Delirium, Depression, and Dementia are some of the most common psychological diagnoses in the elderly today. The three D’s are difficult to differentiate between in older adults because they overlap with each other and can all exist in the same patient at once. Delirium, Dementia, and Depression all affect the elderly’s quality of life and often increase the risks for one another (Downing, Caprio & Lyness, 2013). For the purpose of this paper I will be focusing primarily on the diagnosis of Dementia, the prevention, and nursing measures associated with it, but first I would like to differentiate between Delirium and Depression because Dementia is often associated with the two in the older adult population.

The second step is to set a goal between the patient and those involved with the patient’s care. In order for nurses to practice ethically they must understand the meaning behind each professional value. The first value is human dignity. With each patient we encounter we must show them respect as individuals. It does not matter what the medical condition is, race, ethnicity, culture, religion or gender of the individual. The dignity of a person must be protected. As nurses we must respect the uniqueness of each individual we care for, as this is a fundamental right of all humans. A person with dignity feels a sense of self-worth and as nurses we must do our best to help our patients maintain that feeling of worthiness.
