2-2 Journal: Defining Your Selected Public Health Issue When defining a public health issue, it is important to highlight what is already known and examine what may be missing (Community Tool Box, n.d). Diabetes is the chosen public health issues, and although there is a clinical definition that accompanies the condition, the reason why it is a public health issue needs to be defined. But why? The but why technique takes us to look at and describe the community that is being affected by this issue and subsequent whys should allow us to address the underlying determinants of health associated with diabetes. In this journal, I will define diabetes as a public health issue, describe the impact on the community, identify gaps and disparities and establish how social determinants of health impact the issue. Although significant diabetes disparities exist among racial/ethnic minorities in both health outcomes and quality of care, the problem of suboptimal diabetes care is one that exists for both minorities and whites (Peek et al., 2007). Therefore, significant effort in ongoing strategies to reduce or eliminate racial/ethnic disparities in health has been focused on improving delivery, access, and quality of care (Walker, Williams, & Egede, 2016). The public needs to address racial disparities in health which is achievable by changing policy addressing the major components of socioeconomic status (income, education, and occupation) as well as the pathways by which these affect health. To modify these risk factors, one needs to look even further to consider the factors. Socioeconomic status is a key underlying factor. Several components need to be identified to offer more options for those working on policy making. Because the issue is so big, I believe that not a single policy can eliminate health disparities in the United States. One possible pathway can be education, like the campaign to decrease tobacco usage, which is still a big problem, but the health issue has decreased in severity. Health disparity is one of the burdens that contributes to our healthcare system in providing equal healthcare to everyone regarding of race, age, race, sexual orientation, and socioeconomic status to achieve good health. Research reveals that racial and ethnic minorities are likely to receive lower quality of healthcare services than white Americans.
Diabetes is a prevalent health disparity among the Latino population. Diabetes is listed as the fifth leading cause of death among the Latino population in the website for Center for Disease Control and Prevention, CDC, in 2009. According to McBean, “the 2001 prevalence among Hispanics was significantly higher than among blacks.” (2317) In other words among the Hispanic or Latino community, there is a higher occurrence of diabetes as compared to other racial/ethnic groups such as Blacks and Native Americans. The prevalence of diabetes among Latinos is attributed to the social determinants of health such as low socioeconomic status and level of education. Further, this becomes an important public health issue when it costs the United States $174 billion in both direct and indirect costs, based on the 2007 The National Diabetes Fact Sheet released by the CDC. In turn, medical expenses are twice as high for a patient that has diabetes as opposed to one without. Finally, this high cost becomes another barrier to receiving care for Latinos when some are in the low socioeconomic status.
Diabetes is a disease associated with the inability to store and metabolise glucose effectively. In the UK alone there are just under 3 million people diagnosed with diabetes and approximately 850,000 who are not aware that they have it. (Nhs.uk, 2014)
The disparities in the healthcare system contribute to the overall health status disparities that affect ethnic and racial minorities. The sources of ethnic and racial healthcare disparities include cultural barriers, geography differences, or healthcare provider stereotyping. In addition, difficulties in communication between health care providers and patients, lack of access to healthcare providers, and lack of access to adequate health care coverage
Healthcare disparities are when there are inequalities or differences of the conditions of health and the quality of care that is received among specific groups of people such as African Americans, Caucasians, Asians, or Hispanics. Not only does it occur between racial and ethnic groups, health disparities can happen between males and females as well. Minorities have the worst healthcare outcomes, higher death rates, and are more prone to terminal diseases. For African American men and women, some of the most common health disparities are diabetes, cancer, hypertension, cardiovascular disease, and HIV infections. Some factors that can contribute to disparities are healthcare access, transportation, specialist referrals, and non-effective communication with patients. There is also much racism that still occurs today, which can be another reason African Americans may be mistreated with their healthcare. “Although both black and white patients tended not to endorse the existence of racism in the medical system, African Americans patients were more likely to perceive racism” (Laveist, Nickerson, Bowie, 2000). Over the years, the health care system has made improvements but some Americans, such as African Americans, are still being treating unequally when wanting the same care they desire as everyone else.

Seeking to position lower socioeconomic status above racial/ethnic biases or vice versa is irresponsible to the goal of eliminating healthcare delivery differences at large. Both these are realities of a group of people who are not receiving the same level of care from the healthcare professionals although they exist within one of the most resource rich countries in the world, the United States. According to House & Williams (2000), “racism restricts and truncates socioeconomic attainment” (page, 106). This alone will hinder good health and spur on disparities as racism reduces the level of education and income as well as the prospect of better jobs. Despite the substantial developments in diagnostic and treatment processes, there is convincing evidence that ethnic and racial minorities normally access and receive low quality services compared to the majority communities (Lum, 2011). As such, minority groups have higher mortality and morbidity rates arising from both preventable and treatable diseases judged against the majority groups. In conclusion, diabetes is a serious disease and can be life threatening. With the right research and implementation of new studies diabetes can be reduced among the populations. We as a people need to realize this epidemic and all get together and beat it. Restaurants need to stop putting chemicals and unnecessary fats in their food. Not only diabetics but everyone should educate themselves on what is going in your body when you eat. Also, exercising and getting off the couch, in children, needs to be addressed and that alone would help reduce diabetes in the younger population. I can’t stress enough on education. If you know what you are eating or how you are exercising you protect yourself from poor health and live a great life. Type 2 diabetes can be dwindled down with a conscious individual and group effort.
Point blank, diabetes is a serious disease and causes major effects on people’s daily lives. In a society where food comes in such abundance, people are overeating.
