The first Nursing Diagnoses is Population Health Promotion. The population expresses a concern, thus they become motivated and want to increase their well-being with an overall desire to maxmimize human health potential. This is evidenced by a willingness to engage in specific health behaviors, that can be used in any healthy condition. Health promotion ideas and behaviors may exist in a single person, a family, or community. The majority of the population has two major health concerns, first one being cancer and the second one is the increase in the geriatric community. The different theoretical concepts during each step to address the thought process and concerns that may arise. This will go over a step by step process on how this plan is going to work and where we are during each stage of the process. Coordinate, through group effort, the services of each department through teamwork to get the maximum independence needed for each individual plan/step. There will need to be an evaluation of health promotion activities that is measureable and defined. Advocate for the different communities and their particular needs and adjust accordingly. Communication to make sure the new plan is working as it should, processes that could be done different and different types of emotional barriers, if any, the patient experienced. The different types of coping skills and the comfort level using each one, this may help reduce anxiety by relaxing the patient into their new roles. With the geriatric community we have to note their new roles in the communities and their comfort levels with those roles. Some geriatric patients are comfortable some are not and we have to pay close attention to their anxiety levels during each stage to ensure coping measures are
