NURS 4376 Population-Focused Community Health is one of the nursing courses that has helped me achieved each of the seven Student Learning Outcomes (SLO). This course was focused on the community and population health and healthcare concerns. The assignment from Module A was about the community survey assessment and analysis of community resources. It had two parts; the first part including driving around the community also known as the community windshield assessment to check the healthcare resources such as community hospitals, clinics, long-term care and nursing rehab facilities including public transportations. I have learned that each health professional and non-professional individuals have important roles and responsibilities to the patient care and safety. Therefore, working in collaboration with all these individuals is crucial to ensure continuous patient care and safety. An effective communication with other healthcare members enhances patient safety and care. However, failure to the effectively communicate patient needs and concerns to other health members may jeopardize patient safety and quality care, also result in patient poor outcomes. Effective teamwork and communication between health-care professionals, caregivers, and patients are essential in continuous safe patient care (Nancarrow, Booth, Ariss, Smith, Enderby, & Roots, 2013).
Provide Safe Care to Individuals, Families, and Communities through Individual Performance and System Effectiveness
I achieved this section through evidence-based research. I have learned from the Texas BON rules and regulations and the nurse practice act (NPA) how to provide safe and competent care to patients, families, and communities. The BON governs the nursing practice and provides guidance for professional nurses for safe practice. Additionally, it has each nurse competencies written according to the degree of study (Texas Board of Nursing, 2013).

I have learned that the implementation of nursing informatics in today’s nursing practice is crucial in enhancing patient quality care, safety, satisfaction, outcomes, and nurse-physician communication. I also learned the importance of nursing informatics use and its benefits in daily nursing practice. According to Yoder-Wise (2015), technology has significantly improved over the years and will continue to influence nursing practice and health care system. Therefore, nurses are encouraged to participate in the leadership position in continuing to enhance patient quality of care and safety through clinical nursing informatics.
Apply Quality Improvement Measures to Continually Improve Health Outcomes Consistent With Current Professional Knowledge
I achieved this section through the NURS 4374-Management and Leadership course activity from Module C assignment. I have learned about the importance of continuous quality improvement, inter-professional team building, and patient safety by completing the root cause and systems analysis activity through the Institute for Healthcare Improvement (IHI). I obtained a PS 201 certificate at the completion of the activity. Nurses should take initiatives and implement quality improvement measures to continuously improve patient quality care and safety (Yoder-Wise, 2015).
