The concept of patient-centered care (PCC), also known as client-centered care or resident-centered care, is described via the Institute of Medicine (IOM) as “providing care that is respectful of and responsive to individual patient preferences, needs and values and enduring that patient values guide all clinical decision (________).” Patient-centered care is the method of tending to patients and their families in a way they will like to be taking care of, and this is done by being respectful, responsive to individual needs, values in every clinical decision made by the clinicians and non-clinicians in promoting health, wellness and disease prevention (__________). The nurse gave the children opportunity to decide whether to send their parent to a skilled facility for rehabilitation or to be discharged home to either one of the children’s homes with a home health agency to follow up with her care. Interdisciplinary care plan meeting between a healthcare staff patient and family members is another example of how patient-centered care(PCC) can be achieved. The patient’s health condition and care should be discussed while still in a health care facility to improve care and to prepare the patient for discharge either to a rehabilitation facility or home with the involvement of the patient and family. The patient's release will be based on the family decision. If the family wants to discharge her home, they will go through health teaching on how to care for the patient at home both in medication administration, nutrition and provision of activities of daily living. To attain positive feedback, the physicians, nurses, other healthcare professions, patients, and their families should be involved in the development of care plan that is measurable. Effective communication between the healthcare professionals, patients, and family members are required to prevent frustrations on the patients and family members.
