Mr. Wilkinson greeted Disability Network staff with enthusiasm, at his home, on the day of the assessment. His hand shake was firm and no hygiene issues were noted. Mr. Wilkinson was given a general overview for the purpose of the assessment: to help with identifying potential barriers to pursuing employment or education. He was encouraged to ask for clarification of questions or statements that were unclear. He confirmed that he was his own guardian and chose to have his father present for the assessment. His father later excused himself, citing the need for him to through the process alone. Trevor Wilkinson is an 18-years-old, Caucasian male. Wilkinson was consistently passionate about fashion and the performing art. He always appears to be relaxed and could be described as optimistic. While it does not appear that he takes deliberate risks, it may be that his awareness is low or he minimized them, which could have implications for certain types of employment. As such, he may benefit from clarify that he understands the consequences/risks associated with specific task. Recommendations: 1. A Medical Evaluation could help with establishing the state of Mr. Wilkinson’s PKU diagnosis, and whether he needs to consult with a dietician. His acid reflux and seasonal allergies could also be address as it could impact employment. 2. Nuero/Psychological evaluation may be helpful as he indicated not following a PKU diet could negatively impact his ability to learn. This may also assist with establishing the state of his ADD, any medications that may have been prescribed, and any implications for employment. 3. Disability Network Capital Area services: a. Transferable Skills training could help with assessing Mr. Wilkinson’s ability to be safe in the kitchen. This class would also assess his sense of space and sequence, and his ability to follow instructions (verbal, written).
