I have spoken with Ms. Tocco many times regarding the massage therapy orders she has. I have researched and provided her with locations close to her home. I did access the HAP web site for locations also. I have confirmed with these locations that they can bill HAP the primary insurance first and then auto for any remaining fees. Ms. Tocco will now call and make appointments. She said she would make the appointments. On 4/26/17 I met Ms. Tocco at the office of Dr. Voci. She arrived to the appointment driven by her Father. Current symptoms are, dizziness, off balance, cervical pain, right arm electrical pain that will radiate down her right leg, low back pain with radiation down the left leg, migraines, and jaw pain. Voci’s physician assistant for a lengthy appointment. The recent EMG of the upper extremities showed mild carpal tunnel to the right hand, there was no radiculopathy from the cervical spine, the EMG of the lower extremities was normal with any radiculopathy of the lower extremities. The VNG for vertigo was negative. A recent MRI of the lumbar spine showed bulges at L3/L4, narrowing, bulges at L4/5, L5/Si. Ms. Tocco again expressed how helpless she feels with her pain, balance, migraines, and jaw pain. It has affected her whole life. She has a lot of anxiety related to her current continued symptoms. She said Dr. Morelli wants her to have some epidural injection, but also told her she would need to have another cervical spine surgery to repair the damage from the original repair not healing. In the meantime, medications are being changed to try and get the migraines under control, Cambia and Maxalt will be tried.
