In the recreation therapy processes phase began with an individual assessment. They need to have confident in themselves, through interest, values in doing leisure activities and the expectations from a recreation therapy. There are resources available in their community and outside. The patient need to learn how to be able to socialize and build up their trust and relationship so it can help them adjust their lifestyles for any type of healthy leisure function. Recreation therapy will use these plan and produces by doing Assessment, planning, implementation and Evaluation to the patient background and improving their health wound benefit them.

When the baseline of the assessment have been completed, the recreation therapist will be able to identify and figure out their patient concern and strength. The assessment is only the beginning to figure out where the patient stands as of right now in order to be place in working with the correct therapy. Recreation therapist must use the assessment in order to understand the patient. They will access useful information to help the patient benefit from their treatments. Assessment only determine the patient strength, interest, expectations and identify the problem or concern. Its will allow the recreation therapist to do a planning phase base on what is given. These four steps in planning phase allow recreation therapy to figure out patient needs, goals, and actions. This will help the patient to move on to the next step of treatment plan.
Goal or general objective is the outcome dealing with the patient behavior. Formulate goal is for the patient to have clear understanding feeling of accomplishment when they achieved it. Specific objective is where the patient has the skill to improve their behavior by themselves rather having a recreation therapy letting their know patient they have made a lot of improvement in the treatment.
The recreation therapy individualized intervention plan will help the patient to be qualify for TR/RT professional (CTRS) based on the result of their assessment in strength, needs, goal, objectives, and intervention outcome. Recreation therapy will write an intervention plan for the patient of what they intend to accomplish in the treatment. The common element would likely be found in individualized intervention plan would be the need list of the patient problem and need, formulate a strength list, goal set, specific objective for each goal, a plan, and evaluation. They need a list to lead into development and goal setting of the patient need. The strength list will be decision making process of a plan in activity
