In November 2010, the Institute of Medicine (IOM) issued a special report titled “The Future of Nursing: Leading Change, Advancing Health” (__). The important issues discussed in the report was how nursing education could be improved to meet the demands of the changing healthcare systems. The reason why IOM is requesting for higher education in the nursing profession is so that future nurses can be known on public health and health policy matters, possess leadership skills and have the abilities to research and apply evidence based on many hours of practicing. Also, more schooling for nurses will help them become more adept at using the new life-saving technologies in the care of patients in the clinical settings. To encourage nurses with associate degrees and high school diplomas to enroll into baccalaureate nursing schools, IOM also recommended expanding loans and grants for nursing students at all levels. Correspondingly, doubling the number of nurses with masters or doctorate degrees by 2020 as well. Such growth in educated nurses will help reduce the shortage of nurses as primary health care providers. The IOM report had four key messages needed for advancing the future of nursing. “Nurses should practice to the full extent of their education and training; achieve higher levels of education and training through an improved education system that promotes seamless academic progression; be full partners, with physicians and other health care professionals, …and; effective workforce planning and policy making require better data collection and an improved information infrastructure” (Institute of Medicine, 2011). The report also included eight recommendations needed to facilitate the necessary changes to in the nursing profession so meet to demands of the healthcare reform.
The national shortage of Registered Nurses (RNs) has helped generate formidable interest in the nursing profession among people entering the workforce and those pursuing a career change. According to a report issued by the U.S. Department of Health and Human Service in 2002, the national population is continuing to grow and age and medical services continue to advance, so the need for nurses will continue to increase. They report from 2000 to 2020 the predicted shortage of nurses is expected to grow to 29 percent, compared to a 6 percent shortage in 2000. With the projected supply, demand, and shortage of registered nurses and nursing salaries ever-increasing, the nursing profession can offer countless opportunities. But first one must determine which educational path to pursue, Bachelor of Science in Nursing (BSN) or Associate Degree in nursing (ADN). Most will initially be educated at the associate degree level, even though the American Organization of Nursing Executives (AONE) has recommended a baccalaureate level as a minimal for entry-level nurses. With the expanding number of RN to BSN programs available there is always the option to further one’s education at a later date. The benefits for acquiring a BSN over an ADN include a better knowledge for evidence-based practice, an increased advantage for promotion, and the necessary gateway for higher education.

This paper focuses on providing a summary of the efforts of the Robert Wood Johnson Foundation Committee Initiative on the Future of Nursing and the Institute of Medicine research which influenced the IOM report, “Future of Nursing: Leading Change, Advancing Health.” The importance of the IOM “Future of Nursing” report related to the nursing practice, nursing education and nursing workforce development will be stated. The role of state-based action coalitions and how they advance goals of the Future of Nursing: Campaign for Action will be described. A summary on will be provided of two (2) initiatives that are spearheaded by Florida state’s Action Coalition with an explanation of ways in which the initiatives advance the nursing profession. Existing barriers to advancement currently in Florida and ways in which nursing advocates may overcome these barriers will be discussed.
The effects on nursing education includes a growth in the number of higher cadre of nurses equipped with leadership and community health skills and knowledge. Institutions with higher trained nurses have recorded less mortality rate (AACN, 2014). The 2010 Institute of Medicine (IOM) Report- The Future of Nursing described the role that nurses have in the current and future US health care environment (IOM, 2011). This report was completed at a time when the Affordable Care Act had been passed and a new emphasis was being put on interdisciplinary healthcare teams, care coordination, value-based payment systems, and preventative care (IOM, 2011). Nursing is the largest profession in health care and with an aging baby boomer populace, the expanded role of nurses will be critical in meeting the growing healthcare burdens (Sisko et al., 2014).
Once upon a time, my best friend, Bryan Martinez, often heard his mother’s medical conversations with friends. One day at school, our teacher confronted Mrs. Martinez and told her that she was able tell that Bryan was a son from a nurse. Apparently there was an incident at school where a little boy was acting out and Bryan told our teacher that the little boy was agitated, and to give him some medication to calm him down. As demonstrated by Bryan, nursing is ongoing profession that promotes the health and well-being of individuals.

The shortage of registered nurses (RNs) in the United States has been a cyclical topic dating back to the 1960s. Only recently have employers in certain regions of the nation stated a decline in the demand for RNs. Consequently, according to the American Association of Colleges of Nursing’s (American Association of Colleges of Nursing [AACN], 2014) report on 2012-2013 Enrollment and Graduations in Baccalaureate and Graduate Programs in Nursing, American nursing schools denied admission to 79,659 qualified applicants from baccalaureate and graduate nursing programs in 2012. The reported decrease in job availability and rejected admissions has left many individuals to question if the nursing shortage still exists. On the other hand, some experts project that the United States will be short more than one million RNs by 2020 (Dolan, 2011). Although some parts of the country are in less of a demand than others, it is undeniable that there is a national shortage of RNs.
Nursing should be a partner, with the medical teams in redesigning the American health care system. Policy making and efficient workforce preparation require advanced data collection and an upgraded information support. Profession of Nursing is always regards as a complex, dynamic and noble. The nursing profession requires many things such as hard work, a vast knowledge, excellent communication skills, and a passion for the noble profession. All these qualities are needed to be an effective nurse. Today, nurses are living in a world of ever changing field of medicine .The present patient statistics poses, nurses are expected to take responsibility of accomplishing the requirements. The requirement for excellent nursing skills is added as well as well-formed skills in different aspect. Because of the high demand in excellent nursing care, the IOM have declared the recommendations for the future of nursing.
There has been a drastic cut in both the Medicare and Medicaid reimbursement rates. According to Robert (2012), “cuts of more than $360 billion to Medicare and Medicaid will be made over the next 10 years. The focus of nursing needs to emphasize more on wellness care and prevention rather than acute care” (McNeal, G., 2012). Nursing practice will need to shift more towards community and population focused nursing. Baccalaureate prepared nurses are provided with a curriculum that includes both community health and leadership skills that are not included in the associate program. By encouraging associate’s degree nurses to obtain a bachelor’s degree in nursing, an increase in the awareness of the needs of the community and population may be seen. The IOM report has outlined the anticipated obstacles that healthcare will face if changes are not made. By allowing nurses to provide care within their full scope of practice, quality care may be provided at an affordable cost to the population. The use of advanced practice nurses in primary care may provide quality, access, and cost efficient healthcare to high-risk populations and possibly decrease hospital admission rates, thus lowering the overall cost of healthcare. Several years ago in 2008 the RWJF and the IOM collaborated for two years to discern the future needs of the nursing profession. Most importantly, the objective was to outline the critical actions needed to ensure nursing was ready to seamlessly move towards the future. This was no easy task as nurses work in such diverse settings such as outpatient areas, acute care settings, the community, and long term settings to name a few. Couple this with the fact that nurses have a variety of educational avenues such as the associates, diploma, or bachelor’s degree open to them to achieve the status of registered nurse (Institute of Medicine, 2010). All of this considered, the committee did design four key messages regarding the future of nursing as key in the transformation of health care as evidenced in their "Future of Nursing" report.
Issues and trends in the nursing profession are constantly changing due to discoveries in technology and research, changes in reimbursement and legislation, and the changing needs of our population; in fact the only constant in healthcare is change. This course has provided me with a foundation and better understanding of the need for higher education to empower nurses to become leaders and help shape the future of the nursing profession. Nurses are the “backbone” of healthcare and this is becoming more evident with the changes we are currently seeing in the healthcare industry.
