Ethic in Healthcare
Ethics is a major branch of philosophy that guides in determining what is right and wrong. Ethical principles are the core elements characteristic of health professionals. Ethical principles important to the health care environment are as follows:
• Beneficence
• Autonomy
• Veracity
• Justice
• Nonmaleficence
• Fidelity
• Confidentiality
These principles are a guide to conduct. Each principle can be applied to the professional interactions of the healthcare provider with patients, colleagues, and community members.
All members of the healthcare team are obligated to practice using ethical principles and codes of ethics for guidelines. Healthcare providers are guided in their decisions by their personal values. These values are influenced by education, personal and professional experiences, and religious and cultural background. The American Nurses Association Code of Ethics provides guidelines to its members for appropriate behavior in various situations and assists in monitoring the professionalism. However, the Code of Ethics provisions “do not focus on giving precise answers to specific ethical problems but, rather, provide general guidance as to how to act when faced with ethical dilemmas” (Huber, 2014, p.275). An ethical dilemma occurs when one duty or obligation is in conflict with another. Ethical dilemmas cannot always be resolved by a code of ethics alone. Every nurse will be faced with a decision making dilemma at some point in his or her career. Being familiar with the nursing code of ethics, what is ethically and morally expected in society and how to approach the situations can help make dilemmas less of a nightmare. “The purpose of nursing ethics is to inspire questions and examine what would be the ethically right action in health care situations demanding a choice between at least two undesirable alternatives” (Toren & Wagner, 2010, p. 394). There are many different ways one can approach a situation to reach a resolution, finding a method that works best with the situation at hand is ideal.

Since ethical dilemmas are not always easily answered through the use of The Code of Ethics, ethical decision-making models are effective tools that can assist nurses in dealing with ethical issues. Ethical decision-making models provide a framework for working through difficult choices. They seek to define the limits of what is morally acceptable and help clarify the guidelines for making those difficult decisions. Ethical decision making models assist nurses in analyzing situations by focusing on understanding the patient needs, need for responsiveness to circumstantial considerations, and recognizing the uniqueness of each situation.
Providing the steps to ethically sound excellent care, healthcare providers must acknowledge first the legal and ethical matters involved with proper investigation and then devise a plan for best possible action recognizing the rights of the patient and its benefits followed by the application of the chosen intervention with positive outcome in mind (Wells, 2007). Delivery of excellent and quality of care at a constant level (NMC, 2008) must be marked in any responsibilities and duties of the care provider to promote exceptional nursing practice. Codes of nursing ethics and legal legislation have addressed almost all the necessary actions in making decisions in consideration to the best interest of the patient. Nurses must make sure that they are all guided by the set standard to lead their action and produce desirable and ethically sound outcomes.
An ethical dilemma is defined as a mental state when the nurse has to make a choice between the options and choices that he or she has at her disposal. The choice is a crucial task as the opting of the step will subsequently determine the health status of the concerned patient, hence it requires a great deal of wisdom along with proper medical and health training before any such step is opted as it is a matter of life and death. Strong emphasis should therefore be on the acquisition of proper knowledge and skills so that nurses do posses the autonomy to interact with patients regarding ethical issues involved in health care affairs and address them efficiently. It is normally argued that nurses are not provided sufficient authority to consult and address their patients on a more communicative or interactive level as a result of which they are often trapped in predicaments where their treatments of action and their personal beliefs create a conflict with the health interests of the patient. Health care ethics is at its most basic definition is a set of moral principles, beliefs and values that guide the Physician Assistant in making choices about medical care for their patients. The central belief of health care ethics is the sense of right and wrong and the principles about what rights we possess, along with the duties as Physician Assistants we owe our patients. (Opacic, D. A. 2003) One must consider carefully all aspects of health care decisions as it helps us make choices that are just.
The classical term for the word ethics is, moral philosophies that rule an individual’s or group behavior or action. The American Nurses Association used ethics to write the Code of Ethics for Nurses with these values and visions in mind: “(1) As a statement of the ethical obligations and duties of every person who chooses to enter the profession of nursing. (2) To act as the nonnegotiable standard of ethics. (3) To serve as an expression of the understanding on nursing’s commitment to society.” (Nurses Code of Ethics, 2015). In this diverse society we are confronted everyday with so many ethical choices in provision of healthcare for individuals. It becomes very difficult to find a guideline that would include a border perspective which might include individual’s beliefs and preference across the world. Due to these controversies, the four principles in biomedical ethic which includes autonomy, beneficence, non-maleficence and justice help us understand and explain which medical practices are ethical and acceptable. These principles are not only used to protect the rights of a patient but also the physician from being violated.
“Ethics is a systematic study of principles of right and wrong conduct” (Taylor, Lillis, Lynn, & LeMone, 2015, p. 96). The American Nurses Association Code of Ethics has nine provisions to it. All nine are important to the nursing field in their own way. There are two provisions that I find most important in helping my career as a nurse. Provision one, The nurse, in all professional relationships, practices with compassion and respect of the inherent dignity, worth, and uniqueness of every individual, unrestricted by considerations of social or economic status, personal, attributes, or the nature of health problems. Secondly provision two, the nurse’s primary commitment is to the patient whether an individual, family, group, or community.
The American Nurses Association created guidelines for the profession including, a set clear rules to be followed by individuals within the profession, Code of Ethics for Nurses. Written in 1893, by Lystra Gretter, and adopted by the ANA in 1926, The Code of Ethics for Nurses details the role metaethics, normative ethics, and applied ethics have within the field (ANA, 2015). Moral obligation for an individual differs within professions than it does within an individual’s personal life, so the code of ethics was written to establish rules within the profession. The moral obligation to provide quality care include the fundamental principles of respect for persons, integrity, autonomy, advocacy, accountability, beneficence, and non-maleficence.
