A community assessment is “an effort to identify and prioritize a community’s health needs, accompanied by collecting and analyzing data, including input from the community” (Clark, 2015, p.357). Community assessments are important in planning to improve and promote health in the community. This community assessment focuses on the community of Lake Andes, South Dakota. Just a little bit about Lake Andes first to get a picture of the community. Lake Andes is home to 879 people. It is located in the southeast corner of South Dakota, on the Yankton Sioux Reservation. Lake Andes residents consist of two predominant races, White and American Indian. The male population consists of: 35 under five years old; 39 5-9 years old; 40 10-14 years old; 23 15-17 years old; 9 18-19 years old; 6 20 years old; 1 21 years old; 14 22-24 years old; 23 25-29 years old; 32 30-34 years old; 24 35-39 years old; 19 40-44 years old; 24 35-39 years old; 19 40-44 years old; 24 45-49 years old; 19 50-54 years old; 20 55-59 years old; 10 60-61 years old; 10 62-64 years old; 7 65-66 years old; 16 67-69 years old; 18 70-74 years old; 16 75-79 years old; 4 80-84 years old; 14 85 years and over. Households are broken down by two types of homes, family and nonfamily homes. The family homes is broken up by ages: 9 15-24 years old; 43 25-34 years old; 43 35-44 years old; 33 45-54 years old; 17 55-59 years old; 10 60-64 years old; 26 65-74 years old; 12 75-84 years old; 2 85 years and older. A windshield survey is “an informal means of collecting population assessment data by driving through an area and observing its visible features” (Clark, 2015, p.359). To address the physical environment, some of the things that look well in the community would be that most of the businesses look well kept, and in the summer time, there are lots of trees and grassy areas. One would not say that air quality looks like it would be an issue. The town does not have any factories or anything that looks like it would really pollute the air. One thing that is concerning is the amount of garbage and trash that a person finds laying around town. If a person drives down the road and looks in the ditches along the highway that runs through town, they are just filled with trash that people throw out of their car
