The community health status assessment is a critical part of the MAPP process where the gathered data will serve as a base for community health analysis, identifying their health issues, and determining where Chicago stands in relation to peer cities, state, and national data. It is appropriately for Chicago Department of Public Health (CDPH) to play a lead role as the community health assessment is one of the ten core function of the public health. The purpose of this phase is to provide the answer for questions such as, “How healthy are Chicago residents?”, “What are the leading causes of illness and death among Chicago residents? How do the lifestyle behaviors of Chicagoans contribute to Chicago’s overall health status? “What are the social, economic, and demographic factors that impact the health and life quality of Chicago community? How does the health status of Chicago compare to that of ten years ago; to that of other communities; to that of the state and the nation? “What are the competencies, activities, components, and capacities of Chicago Department of Public Health?”, and “How are the ten Essential Public Health Services will be provided to Chicago as a community?”. …show more content…

During this process, CHSA, a revision of the health and quality of life data is done by the local public health leaders and the community partners to identify the health problems, resources, strengths, and health care needs for Chicagoans. The results of the CHSA process will provide the MAPP Committee with a better understanding of the residents’ health status and to make sure that the community’s priorities are
