Michael A. Karson was born in Glenside, Pennsylvania. He moved at age one year old to Fort Myers, FL. After 14 years residing in the area the family moved to Virginia for one year. Due to his father employment he moved back to Fort Myers, Florida. He indicated that within the area of Fort Myers he has frequently moved.

His parents separated when he was 15 years old and have been legally divorced for one year. He currently resides with his mother who maintained a relation with a man that died a year ago. The client indicated that he recognizes this person as his stepfather and he was sad to know about his death. He did not reported whether his father has re-married or not. Mr. Karson informed that he has an older sister who is a social worker and lives independently. He also reported a twin brother that resides with his father. According to the information provided by the client, he does not get along well with his mother moreover, he has a good relationship with his father. Mr. Karson has never been married and has no children of his own. …show more content…

She is currently receiving SSDI due to health issues with thyroid and benign cancer. She works part time as a Respite Caretaker. His father holds a Bachelors in Music. He is getting disability due to a diagnosis of Dementia and loss of Motor Skills. The client fraternal brother works as a Manager at Croc’s Shoestore. He is planning to go to Berklee College of Music in Boston, Massachusetts.

The client denied to be working at the time of the evaluation. He stated that he does not have any income. He receives Medicaid to assist with his health care needs. At the time of the interview he was not looking for a
